NFHT has moved to the Port Hope Medical Centre at 249 Ontario St., Suite 209. This is a temporary move to accommodate patients while our offices in the Northumberland Mall are being renovated after a flood in August.
Effective Monday, August 31, 2026, all in-person patients that would normally have happened at the mall will be in the new Port Hope location. There is plenty of parking and accessibility.
Our admin team has been working hard to call patients about the move and we ask for your patience as we work this relocation. We are thankful for the kindness of our community and patients during this challenging period.

The Northumberland Family Health Team is cancelling the COPD Exercise group on Tuesday, Aug. 11 and the RISE group on Aug. 12.

We ask for your patience as we work through assessment and clean-up of our office in the lower level of the Northumberland Mall after it flooded on Aug. 8.

We will continue to post updates here.

Effective August 9, 2026 the NFHT office in the lower level of the Northumberland Mall will be closed due to a flood. The office will remain closed until further notice. Our administrative staff will call all patients with in-person appointments to make other arrangements. Our staff will continue to be working remotely or in other spaces.

We will post updates as we receive them.

The RN will work with our collaborative inter-professional team in assisting patients with the transition from hospital to home or to help in the other direction to keep patients at home instead of in hospital. The successful candidate will bring strong clinical assessment and critical thinking skills, particularly in the areas of Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and chronic disease management.
