It’s Miracle treat day!
Our awesome leadership team here at the Green Clinic sponsored blizzards for 21 of our team members who were hard at work today. It’s not too late to treat yourself to a blizzard, net proceeds will be donated to our local children’s hospital foundation.

Forms

If you have a form you need a practitioner to complete, an appointment is required either in office or by telephone. We ask that you bring or send in the form to the clinic in advance of the appointment filled out as completely as possible and with authorizations signed to avoid delays and save time in the scheduled appointment.
