Is it failure to progress or is it simply failure to wait?

While true failure to progress can occur, it's rather rare.

Birth is a complex hormonal process. Sometimes a stall is environmental. Sometimes a stall is baby working their way into a better position. There are times the body simply needs rest, nourishment, and time.

If mom and baby are safe and tolerating, and there aren't extenuating circumstances, often all that's needed is time.

We are NOT saying you can't use it as a guidelines or to educate parents this might be about the time you'd arrive. And we aren't suggesting it's common to be needed or that all parents should expect you sooner.

But as doulas, I think it's important to know normal labor doesn't always follow patterns.

And to recognize that support needs vary.

If the goal is an empowering birth that feels supported, some parents might need contact sooner. This isn’t because of lack of prenatal prep, but rather is there previous birth trauma? A medically necessary induction after planning for a home birth? Back labor because of baby's position?

And what about multips with a precipitous birth? Where one goes from 4cms to baby in 2 hours - when would she call?

Yes, a general rule is arriving in active labor is good. And a "watched pot never boils" can absolutely be a thing.

But what sets doulas apart from the medical side of maternity care is our ability to provide individualized, holistic support.

If we aren't offering individualized support, what are we offering that's different from the medical model of care?
