Dry needling vs Acupuncture for musculoskeletal pain

I often get asked what is the difference between Acupuncture and dry needling? The differences are extensive in terms of education, training, philosophies, techniques, and application. I will distil this down to what I believe is the key points from the client perspective.

Both, normally use solid filiform acupuncture needles, though I have heard that hypodermic needles may also be used in dry needling.
Dry needling mainly treats musculoskeletal situations by inserting needles into myofascial trigger points (TrPs) - hyper irritable contraction knots - with the aim of releasing muscle tension and pain. This is generally viewed as a local treatment where needling is in the areas of pain.

In sports Acupuncture, these TrPs are also known as ā shì points, which we are trained to locate and needle. With acupuncture, we use distal points and adjacent points which are locations away from the area of pain. The purpose of this is to stimulate the body’s release of natural painkillers (endorphins), anti-inflammatory substances and in many cases the release of muscle tension and pain associated with the initial TrPs point. The acupuncturist also considers the treatment of such items as stress, anxiety and/or insomnia that may be part of the musculoskeletal issue. The aim is to promote a rounded and balanced healing. This leads to a gentler treatment with only mild discomfort association to the insertion of the needles.

Using acupuncture for post surgery care:

Acupuncture and Pre and Post Surgery Care:

Acupuncture can help strengthen your immune system so that you go in for surgery as planned and recover as quickly as possible. It will decrease postoperative pain and side effects.

Getting acupuncture after surgery can help relieve postoperative pain by activating the body’s natural painkillers (such as endorphins and enkephalins), reduce inflammation by modulating the immune system, and improving blood circulation thereby, speeding up the healing process by stimulating the regeneration of tissues and nerves.

Additionally, acupuncture can help with reducing stress, anxiety, and depression, plus help improve the quality of sleep, appetite, and digestion which are all essential for healing.

I have had good results from clients undergoing knee surgery, hip surgery, and postpartum care.

Try acupuncture for pain management

Acupuncture and peripheral nerve injury: remodeling of nerve system.

Peripheral nerve injury (PNI) caused by compression, traction, ischemia, and laceration can cause motor impairment of the limb, disuse atrophy of the limb muscles, and other nutritional disorders and sensory disturbances in the damaged innervated areas. “Acupuncture has its unique advantages in that acupuncture plays an important role in anti-inflammation, analgesia, and speeding up nerve repair. The sequence of postoperative rehabilitation treatment of patients can be acupuncture first and rehabilitation manipulation later, to reduce the pain during rehabilitation manipulation and maximize the analgesic efficiency of acupuncture.” 1

“Research on the mechanism of acupuncture in peripheral nerve injury is currently mostly concentrated at the level of individual molecules and/or signaling cascades. Acupuncture has the potential to control a complex network of several signaling molecules and pathways due to the wide range of interaction communication that exists between various signaling pathways.” 1 In acupuncture we would just say moving qi.

The research does compare manual acupuncture and electro-acupuncture. With manual acupuncture, the focus is on obtaining the de qi sensations. With electro-acupuncture, the focus is on stimulation induced by frequency, current and pulse width. Research has not shown one method to be superior to another. In my practice I use both methods, which is determined at the time of treatment and the responses from the client.
