A mallet finger is a disruption of the terminal tendon on the top of your finger which is responsible for straightening the end joint. This results in a droopy fingertips. In some cases, mallet fingers can lead to a swan neck deformity where the end joint droops and the middle joint hyperextends (bends backwards). This is usually caused by too much pull from the extensor tendon on the middle joint since the overall balance of forces is disrupted.

Treating a mallet finger that has a secondary swan neck deformity requires a difference treatment approach as restoring joint position and tendon balance at both joints is essential. We apply a mallet splint secured with a digisleeve and add a dorsal blocking splint to prevent the middle joint from hyperextending. It can look intimidating and bulky but it is essential to a successful outcome.

Silver ring splints offer an esthetically pleasing option to individuals needing long term finger or thumb joint support. A proper fitting by a skilled therapist is crucial to ensure proper fit once you receive the final product.

Step 1: First off, a thorough assessment is needed to determine your needs and suitability of a silver ring. Cheaper options are often considered first as a trial before committing to a more expensive silver ring.

Step 2: Reach works with two companies who specialize in the fabrication of custom silver ring splints. Depending on the ring(s) you require, your therapist may recommend one company over the other, but you can always choose your preferred company.

Step 3: Each company provides us with precise fitting tools (blue rings on the photo) to customize the fit in your ring(s). The fitting can be time consuming, especially when multiple rings are needed, as our therapists take their time to ensure they find the best possible sizing for your specific needs. Once the fitting is completed, we send your measurements to the fabricant and wait for the rings to arrive.

Ring splints can be a small but powerful tool in a hand therapist's toolbox. They can either be made in clinic from low temperature thermoplastic, pre-fabricated plastic or custom-measured silver rings. The choice of material will depend on what the ring is used for.

If used as part of treatment for an acute injury (trigger finger, ligament injury, tendon injury), a plastic ring (custom thermoplastic or pre-fabricated) will be a better choice as they are readily available and have a low cost. When rings are used as a long-term solution to finger hypermobility, finger deformities and/or arthritis, silver rings can be considered as they are more durable, more comfortable and more esthetically pleasing but they do come with a higher cost.

A thorough assessment of your specific symptoms and needs with your hand therapist will lead you to the right ring(s) to fit your lifestyle.

Thumbs are the most common type of trigger finger in the pediatric population. Contrarily to adults, it is not an overuse injury but a congenital predisposition which usually presents itself before age 3. Conservative treatment via splinting can be successful in mild cases but once the thumb is locked in position, surgery is often required. Hand therapy can support recovery afterwards with splinting and scar management.

Stenosing tenosynovitis, more commonly known as trigger finger, is an overuse condition frequently treated by hand therapists. Conservative treatment is generally successful, with few cases requiring corticosteroid injection or surgery. Treatment is quite simple with immobilization via splinting being the cornerstone of a successful outcome.
