What Does Hair Transplant Surgery Look Like?

This glimpse into what happens during your hair transplant surgery is just a fraction of the planning that goes into each and every hair transplant. Before surgery, our team of skilled nurses walk you through the day and tell you what to expect and what to do. Post surgery, you’re given careful aftercare instructions to make sure the most important part, the healing, goes as it should.

In the days following your surgery, you’ll have follow up appointments with our nurses to gently wash your scalp and remove any scabbing. The end result, after at least 12 months, is your new healthy hair grafts growing in their new places.

The Clinical Reality of TRT and DHT Blockers

For patients undergoing Testosterone Replacement Therapy (TRT), a common clinical concern is the acceleration of androgenetic alopecia (male pattern baldness).

This leads to a frequent clinical question: Can you safely co-administer 5α-reductase inhibitors like Finasteride or Dutasteride alongside TRT to protect the hair follicle?

The concern for many is whether blocking Dihydrotestosterone (DHT)—a highly potent androgen—will inadvertently blunt the therapeutic benefits of testosterone on muscle mass, strength, and libido.

In our latest video, Dr. Sharma reviews a landmark, five-year randomized controlled trial published in JAMA (The Journal of the American Medical Association) that investigated this exact interaction.

The Clinical Dilemma
When exogenous testosterone is introduced, the 5α-reductase enzyme converts a portion of it into DHT. While DHT is a primary driver of hair follicle miniaturization and prostate enlargement, its precise role in mediating testosterone's positive effects on skeletal muscle, erythropoiesis, and sexual function has historically been poorly understood.

If DHT is the primary mediator of these physiological benefits, blocking its conversion should theoretically attenuate the efficacy of TRT.

The JAMA study evaluated healthy men receiving Testosterone Enanthate paired with either a placebo or Dutasteride (a powerful dual-inhibitor of 5α-reductase).

The findings offer critical, evidence-based insights for anyone looking to optimize their hormone therapy while managing dermatological side effects. Dr. Sharma breaks down the data, the physiological outcomes, and how we approach this balance clinically at CHTC.
