Congratulations Espanola Family Health Team 🎉🥳👏
A Major Milestone for Primary Care in Espanola!
We are proud to share that the Espanola Family Health Team has successfully met a provincial mandate to connect all individuals who were registered with Health Care Connect prior to January 2025 with a primary care provider or care team.
Through the dedicated efforts of our team and healthcare partners across the region, 151 new patients have been successfully rostered with the Espanola Family Health Team, ensuring they now have access to ongoing primary care services.
This achievement is an important step toward the Province of Ontario’s goal of ensuring that every resident has access to primary care by 2029.
Recognizing the Team Behind the Success
Pictured is the Primary Care Access Team, whose leadership and dedication were instrumental in achieving this milestone:

• Kelly Joyce – Medical Receptionist
Their work in patient outreach, assessments, intake processes, and patient attachments helped connect residents with the primary care services they need.
