🤒 𝗡𝗼 𝗼𝗻𝗲 𝘄𝗮𝗻𝘁𝘀 𝘁𝗼 𝗴𝗲𝘁 𝘀𝗶𝗰𝗸 𝗳𝗿𝗼𝗺 𝗮 𝘁𝗶𝗰𝗸.

You can be bitten by a tick and not even know it. Symptoms of tick-borne diseases don’t always show up right away—they can take 𝟯 𝘁𝗼 𝟯𝟬 𝗱𝗮𝘆𝘀 to appear.

Illnesses like Lyme disease, anaplasmosis, babesiosis and Powassan virus can develop days or even weeks after a bite from an infected tick.

⚠️ 𝗪𝗮𝘁𝗰𝗵 𝗳𝗼𝗿 𝗲𝗮𝗿𝗹𝘆 𝘀𝘆𝗺𝗽𝘁𝗼𝗺𝘀: fever, fatigue, headache, muscle aches, nausea or a rash.

If you’re feeling unwell and have spent time in places where ticks live—like wooded or grassy areas—don’t ignore it. Contact your local public health unit or speak to your health care provider.

Today, we welcomed Wilfred Cheung, Chief Strategy, Transformation and Engagement Officer at Ontario Health atHome, for a visit hosted by the Guelph Wellington OHT. The visit highlighted the power of partnership in delivering coordinated care closer to home through the Integrated Patient Care Team (IPCT) Home Care Program.

Through conversations with Team Members and patient-centred stories, the visit highlighted how home care and primary care providers are working together as one integrated team to better support people receiving care at home.

Since launching in January 2025, the IPCT Home Care Program has supported more than 1,040 patients across Guelph Wellington. The program is helping people access services more quickly, receive more coordinated care, and build the confidence to manage their health and independence at home.

Guelph FHT was an early adopter of the patient-centered team-based home care model and currently operates five IPCT sites.

We're grateful for the opportunity to share the successes, lessons learned and ongoing evolution of this innovative model of care. Thank you to Wilfred for taking the time to visit and learn more about the impact of collaboration in action.

This week, we celebrated an exciting milestone for healthcare in our community with the official launch of the new Family Medicine Residency Training Site in Guelph.

While this new site will welcome its first residents on July 1, its impact will be felt for years to come. Training future family physicians within the communities they serve is one of the best ways to strengthen access to primary care and build a sustainable healthcare workforce.

Residents will learn alongside our interdisciplinary healthcare teams, build relationships with patients and providers, and become part of the fabric of our community. We hope many will choose to stay and practice here, helping meet the healthcare needs of Guelph and Wellington County for generations to come.

Thank you to our partners at McMaster University and everyone who helped bring this vision to life. We are grateful to our physician leaders, staff, learners, community members, and elected officials, including Mike Schreiner and Leanne Caron, who joined us to celebrate this important milestone. Together, we are helping build a stronger future for primary care.

Today is Earth Day and marks Guelph FHT’s 4th Annual Green Health Day🌎🌱🌿

Green Health Day highlights the critical connection between environmental change and human health. Protecting the planet means protecting ourselves, and we all have a role to play in driving positive change. Climate change is a growing public health concern, causing extreme heat, poor air quality, and tick and mosquito-borne illnesses to become more common in our community.

We want to take a moment to reflect on the role we can play in creating healthier, more sustainable environments, for all. We encourage everyone to wear green today and to learn more about how we can work together to make sustainable changes.
