When most people think of therapy, they picture lying on a couch talking about their childhood.

That's not what trauma therapy for first responders looks like.

Effective trauma therapy is focused and structured. We work on what's actually happening in your nervous system — the hypervigilance, the emotional numbness, the disconnection — not just talking about it endlessly.

I also want to be honest about something: to work through trauma, you will need to think and talk about what happened. That can stir up feelings you've worked hard to suppress. It may feel like things are getting harder before they get better. That's a normal part of the process — not a sign that therapy isn't working.

The first session is simply getting to know each other. No pressure to go straight into the hard stuff.

There's something that happens to first responders that doesn't always get called by its name.

You did everything right. Or you tried. But someone didn't get help in time. Or the system failed. Or you witnessed something that no person should ever have to witness.

And now you carry it — the guilt, the shame, the feeling that you should have done more.

That's not weakness. That's moral injury. And it's different from PTSD — and just as real.

They describe it as: can't switch off. Snapping at the people they love. Using alcohol to wind down. Feeling numb. Avoiding the news. Dreading the next call.

These aren't personality changes. They're symptoms. And there's a difference between work stress and PTSD — one that matters for how you get help.

There's something that affects a lot of first responders that doesn't always get talked about — or even named.

It's called moral injury.


It's not the same as PTSD. PTSD is driven by fear and the sense of ongoing danger. Moral injury is driven by guilt, shame, and betrayal — the feeling that you should have done more, or that the system failed someone who needed help.


Both are real. Both are treatable. And both deserve proper support.
