Aging, Community and Health Research Unit Aging, Community and Health Research Unit

The aim of this research program is to promote optimal aging at home for older adults with multiple chronic conditions (MCC) and to support their family caregivers. To this end, the program will design, evaluate and translate new and innovative inter-professional community-based interventions to improve access to health care, health-related quality of life, and health outcomes in this population, while reducing costs.

In the second video, our Patient Partners discuss their vision for and potential impact of the My Stroke Recovery Journey website. The website was part of the Transitional Care Stroke Intervention. This interactive website, www.mystrokerecoveryjourney.ca was co-designed with patient partners, and produced by the Aging, Community and Health Research Unit (ACHRU), and with support from the McMaster Collaborative for Health and Aging, to support system navigation for older adults with a stroke transitioning from hospital to home.
