Before we can build better care, we must listen. The FLA OHT is committed to learning from the lived experiences of Indigenous community members to ensure palliative care services in our region are supportive, respectful, and culturally safe.
As part of an ongoing community-based study led by Dr. Amrita Roy and Dr. Sarah Funnell, we are inviting Indigenous community members to join us in a Talking Circle.

If you, a family member, or a loved one has experience with or a need for palliative care, we would love to hear your story. Your insights will gather important information to help improve how end-of-life care is delivered.

At the FLA OHT, we believe that the future of health care shouldn’t just be designed for young people—it should be designed with them. Your lived experiences, fresh ideas, and unique perspectives make you a natural leader.
Are you a youth, young adult, or student looking to make a real difference in our community? We want you to volunteer with us!

Your voice matters .
(La version française suit)

Dawn Smallridge has spent her career showing up for people at their hardest moments. As a personal support worker, end-of-life doula and grief coach, she knows what it feels like to navigate a system that doesn't always make it easy.
That's why she joined our Community Council.

"I have seen firsthand where the system works well, but also where people can fall through the cracks. I believe lived experience matters, and I wanted to help make sure community voices are truly being heard."

Her message to anyone thinking about getting involved: "Your voice matters more than you think it does. You do not need to be an expert to make a difference."

Health Care Connect is a provincial program designed to help match you with a primary care provider in your local community who is accepting new people.
How it works:
1ïžâƒŁ Register: Sign up online or by phone.
2ïžâƒŁ Get Matched: A Care Connector in our region will work to connect you to a local provider when space is available.
3ïžâƒŁ Connect: You’ll be notified with next steps.

$3.62M to connect communities to care across FLA

(La version française suit)

The FLA OHT, through our partnership with the Maple Family Health Team, has received $3.62 million from the Ontario government to connect up to 8,500 people to a family physician or nurse practitioner by the summer of 2027.

This builds on the 4,250+ people we helped attach to primary care between April 2025 and March 2026, and it means more recruitment, another community hub, and stronger local health teams in communities like Sharbot Lake, South Frontenac, Northbrook and Napanee.

For everyone who has been waiting for a family doctor, this is real progress.
Thank you to the Ontario government, the Maple Family Health Team, the Sharbot Lake Family Health Team, and all our partners who made this possible.

Le FLA OHT, en partenariat avec l'équipe de santé familiale Maple, a reçu 3,62 millions de dollars du gouvernement de l'Ontario afin de connecter jusqu'à 8 500 personnes à un médecin de famille ou à un infirmier praticien d'ici l'été 2027.

Cela s'appuie sur des efforts dĂ©jĂ  dĂ©ployĂ©s oĂč nous avons aidĂ© plus 4 250 Ă  s'inscrire auprĂšs d'un professionnel de soins primaires entre avril 2025 et mars 2026.

ConcrÚtement, cet investissement servira à réaliser davantage de recrutement, un nouveau centre communautaire et des équipes de santé locales renforcées dans des communautés comme Sharbot Lake, South Frontenac, Northbrook et Napanee.

Pour toutes celles et ceux qui attendent un médecin de famille, c'est un progrÚs concret.
