Early treatment failure after CAR-T: can we identify patients at risk?
A new study of patients with relapsed/refractory multiple myeloma treated with ciltacabtagene autoleucel (cilta-cel) found that:
• Prior BCMA-directed therapy, extramedullary disease, and CAR-HEMATOTOX ≥2 were independent predictors of early progression or death.
• MRD positivity at 3 months was associated with a significantly higher risk of early treatment failure.
• Less than complete response on FDG PET/CT at 3 months was also strongly associated with early treatment failure.
These findings highlight the potential value of early and sensitive disease monitoring after CAR-T, particularly for identifying patients who may need closer follow-up or new therapeutic strategies.
