Can this increase "stress on the knee" ?

Absolutely. But stress and damage are not the same thing.

I have a real problem with the assumption that stress makes an exercise bad.

In fact, it's exactly contrary to how we use stress (load) in rehab & training

The problem isnโ€™t that the knee moves forward.

The problem is when the demands exceed the current capacity of the tissues.

Thatโ€™s an important distinction.

Avoiding these positions forever doesnโ€™t prepare the knee for those demands.

Progressively exposing it to them does.

Because rehab isnโ€™t about avoiding stress.

Itโ€™s about building the capacity to handle it.

With persistent knee pain, people often develop protective movement strategies.

They unload the painful area.

They avoid certain ranges of motion.

They change the way they recruit muscles.

The problem?

Those movement adaptations donโ€™t always disappear when the pain does.

Thatโ€™s one reason recurrence is so common.

This is where externally paced exercise becomes interesting.

Instead of allowing the client to choose their own tempo, movement is paced using an external cue such as a metronome.

Research suggests that combining strength-based loading with externally paced movement may help address some of the motor control adaptations associated with persistent tendon pain. (Ebonie Rio et al. (2015) โ€” Tendon neuroplastic training: changing the way we think about tendon rehabilitation.)

The goal isnโ€™t simply to strengthen the muscles.

Itโ€™s to improve motor control and restore more efficient movement patterns.

Because successful rehab isnโ€™t just about changing tissues.

Itโ€™s about changing how people use them.

And not all โ€œcore exercisesโ€ solve the same problem.

An extension bias occurs when an individual consistently relies on lumbar extension as a strategy to create stability and overcome load.

Instead of distributing force efficiently through the hips, thorax and pillar, they repeatedly ยซ find ยป stability by hinging through the low back.

The result is often excessive stress on the low back despite having adequate strength elsewhere.

For these clients and patients, the goal often becomes improving extension control, and teaching the hips and thoracic spine to contribute more effectively.

Thatโ€™s why exercise selection matters.

The goal isnโ€™t to eliminate extension.

Itโ€™s to improve how force is distributed through the system so the low back doesnโ€™t always have to pay the price.

ARE YOU JUST COLLECTING EXERCISES?

The same exercise can be:

โœ… exactly what someone needs

or

โŒ completely inappropriate

depending on movement bias, symptom behaviour, load tolerance and phase of rehab.

Exercises arenโ€™t good or bad. They're tools.

And tools only make sense within the program.

Instead of asking:

What's the best exercise?

Ask:

What behaviour am I trying to change?

๐Ÿ‘‰Someone with a flexion bias may benefit from a program that improves extension awareness and strength, reinforces flexion control and progressively loads the posterior chain

๐Ÿ‘‰Someone with an extension bias needs better extension control while improving the contribution of the hips and thoracic spine to better distribute force throughout the system

๐Ÿ‘‰Someone who demonstrates poor frontal plane stability may benefit from exercises that improve pelvic control, glute med function and the ability to manage load during single-leg tasks.

Different problems.
Different programs.

The best rehab programs aren't built around exercises.

They're built around movement behaviour.

In fact, if your client or patient already lives in flexion, they may need the exact opposite.

A flexion bias occurs when the lumbar spine moves excessively into flexion relative to the hips and thoracic spine under load.

Common examples:

โ€ข rounding during hinges
โ€ข prolonged sitting intolerance
โ€ข pain with bending forward
โ€ข loss of lumbar position under load

The solution isnโ€™t randomly strengthening the core.

Itโ€™s identifying the movement behavior that needs to change.

For a flexion-biased client, that may mean reinforcing extension awareness and strength.

Because rehab programming should be driven by movement presentation, not generic exercise selection.

Yet many people spend months (or years) trying to protect their low back from stress.

The problem?

Tissues that are never challenged don't become more resilient. They become less prepared for the demands of training, work and everyday life.

That's why a good rehab program should eventually include direct loading of the low back.

Not necessarily by moving the torso on the hips with exercises like RDLs, Goodmornings or Deadlifts.

But sometimes by doing the opposite.

๐Ÿ‘‰ Moving the hips while the torso remains fixed.

Exercises like reverse hyperextensions and straight-leg glute bridges teach the low back to work isometrically while the hips move around it.

This can be helpful when movement is irritable or when there is fear of movement or apprehension.

Because one of the biggest mistakes I see in rehab is treating the low back as something that should never be stressed.

Lewis, Sahrmann & Moran (2007) showed that decreased glute contribution during hip extension increases anterior hip joint force.

Thatโ€™s HUGE for supporting a strategy that focuses on improving not only hip flexion range of motion, but posterior/lateral hip contribution to reduce anterior hip stress.

Because the goal isnโ€™t just to make the hip stronger. Itโ€™s to improve how the system controls and distributes force in positions that were previously painful or poorly tolerated.
