A “tension nose” is an anatomical pattern, not a one-size-fits-all diagnosis.

When the septal cartilage creates forward tension, it can influence how the bridge, tip, and upper lip relate to one another.

In this case, the assessment considered:

→ Dorsal fullness
→ Tip projection and position
→ The relationship between the tip and upper lip

Rhinoplasty planning is individualized. The anatomy guides the approach.

—

L’anatomie guide l’approche.

Why do I ask you to smile in your before-and-after photos?

Because a face is not static.

A photo at rest shows only part of the picture. Smiling, speaking, and making expressions reveal how the nose moves with the rest of the face.

One of the details I assess is the nasal tip. If it drops noticeably with a smile, it gives me information about the relationship between tip support and the muscles around the nose. That movement may influence the surgical plan and the type of support required.

This is why dynamic photos and videos matter. A rhinoplasty should be evaluated not only when the face is still, but also in motion.

Not every neck concern is about superficial fat.

Before planning a deep plane neck lift, I assess four key structures:

→ the platysma
→ the digastric muscles
→ deep central fat
→ the submandibular glands

Each structure can contribute differently to the neck contour.

In this case, the plan focused on platysma support and the digastric muscles. Deep fat was minimal, and the submandibular glands were small and well positioned, so they were not treated.

Individual anatomy guides the plan.

Three years tells a more complete story.

Early post-operative photos are part of the healing process. Long-term follow-up lets us see how the nose settles, refines, and remains in balance with the rest of the face.

Before surgery, this patient had a dorsal hump, increased nostril show, and a tip that was both overprojected and droopy. Together, these features drew attention to the centre of her face, particularly in profile.

The plan was not simply to reduce the bridge. It was to create a more balanced relationship between the dorsum, tip projection, tip position, and nostril visibility.

At three years post-op, the tissues have had time to settle. This is why long-term follow-up matters: the goal is a result that remains harmonious over time, not just in the first few months of healing.

The jawline and neck are anatomically connected.

When assessing the lower face, I also consider:

→ Skin and tissue beneath the chin
→ The platysma muscle
→ Deeper neck structures
→ The relationship between the neck and lower face

Depending on the individual anatomy, a plan may focus on the neck alone or assess the face and neck together.

Individual anatomy guides the plan.

A septoplasty is meant to help you breathe better, not change the appearance of your nose.

But the septum does more than divide the two sides of the nose. It also helps support the bridge and tip.

If too much septal cartilage is removed, the bridge can lose some of that support. Sometimes the change is subtle, like a slight dip through the bridge. In more significant cases, it can lead to what we call a saddle nose deformity.

This is not the expected outcome of a septoplasty. But it is why I do not think of septoplasty as simply “taking cartilage away.” What remains is just as important.

The L-strut is the part of the septum left behind to support the nose. If someone has had a septoplasty in the past, I approach a future rhinoplasty as a revision case. There may be less septal cartilage available, and I may need cartilage from the ear or rib to rebuild support.

Breathing, support, and appearance are all connected.

A nose that appears long in profile is rarely about one feature alone.

In this case, several details were working together: a deep radix, a subtle pseudo dorsal hump, an overprojected tip, and a plunging tip position.

The deep radix made the small hump appear more pronounced. The projection and tip position added to the overall impression of length. Simply reducing the bridge would not have addressed the full relationship between these structures.

The plan was designed from the radix to the tip:

→ Preserve and reposition her parallel dorsal aesthetic lines
→ Augment the deep radix with a mastoid fascia graft, using her own fascia from behind the ear
→ Refine projection and support the tip in a more balanced position

At one month, swelling remains visible in the front view. The profile structure is already taking shape, and definition will continue to settle with time.
