PROGRESS IN MS TREATMENTS: Patients like Pierre Dubé who have been followed at The Neuro for decades have benefited not just from exceptional care in Canada’s oldest MS clinic but from improvements in therapies made possible by years of clinical research at the Neuro CRU.

When the first clinical trials were launched at the Neuro 40 years ago, it was because our Multiple Sclerosis (MS) neurologists wanted their patients to have access to the earliest emerging therapies they hoped would help manage the disease. These treatments went on to be the first approved medications for MS.

The progress in treatments since then has been remarkable: modern therapies promise a future with few, if any, relapses for many newly diagnosed patients.

DES PROGRÈS DANS LES TRAITEMENTS POUR LA SP: Des patients suivis au Neuro depuis des décennies, comme Pierre Dubé, ont bénéficié non seulement de soins exceptionnels dans la plus ancienne clinique de SP du Canada, mais aussi des progrès thérapeutiques rendus possibles par des années de recherche clinique au Neuro CRU.

Il y a 40 ans, lorsque les premiers essais cliniques ont été lancés au Neuro, nos neurologues spécialisés en sclérose en plaques (SP) souhaitaient que leurs patients aient accès aux traitements émergents les plus récents, dans l’espoir qu’ils améliorent la prise en charge de la maladie. Et ces traitements se sont avérés être les premiers approuvés pour la SP.

Les progrès réalisés depuis sont remarquables : les thérapies modernes promettent un avenir avec peu, voire aucune, rechute pour de nombreux patients nouvellement diagnostiqués.

SCLÉROSE EN PLAQUES : LUTTER CONTRE LA PROGRESSION INACTIVE. Il y a à peine dix ans, il aurait été difficile d'imaginer à quel point la vie de nombreuses personnes atteintes de la sclérose en plaques (SP) allait s'améliorer. Les traitements modernes permettent de réduire considérablement les poussées, ce qui permet à de nombreuses personnes de conserver une bonne qualité de vie pendant des années après le diagnostic.

Mais pour les personnes atteintes de la SP dont les symptômes et le handicap s'aggravent malgré les traitements actuels, il est essentiel de trouver de nouvelles approches.

Plusieurs nouveaux essais cliniques au Neuro CRU étudient actuellement si les traitements utilisant des inhibiteurs de la BTK peuvent réduire l'inflammation cérébrale persistante chez ces patients.

MULTIPLE SCLEROSIS: TACKLING INACTIVE PROGRESSION
Just a decade ago, it would have been difficult to imagine how dramatically outcomes for many people with Multiple Sclerosis (MS) would improve. Modern therapies can greatly reduce relapses, allowing many individuals to maintain a high quality of life for years after diagnosis.

But for people whose symptoms and disability worsen despite current therapies, finding new approaches to treat MS is critical.

Now, several new clinical trials at the Neuro CRU are investigating whether treatments using BTK inhibitors can address ongoing inflammation in the brain more effectively than standard of care.

SCLÉROSE EN PLAQUES - DÉPISTER AVANT LES PREMIERS SYMPTÔMES : Les traitements contre la sclérose en plaques (SP) ont progressé au cours des dernières décénnies, au point où les personnes diagnostiquées aujourd’hui auront peu de symptômes ou de rechutes après leur première crise. Si on pouvait dépister les personnes à risque, ces thérapies pourraient stopper les dégâts neurologiques qui mènent au développement de la maladie.

C’est l’objectif du neurologue et chercheur Dr. Adil Harroud au Neuro. Son labo a déceler un groupe de protéines présentent dans le sang de personnes qui ont par la suite développé la SP – jusqu’à 10 ans avant leurs premiers symptômes.

La prochaine étape sera de concevoir des tests sensibles et fiables qui permettent de dépister les personnes à risque le plus tôt possible.

MULTIPLE SCLEROSIS - DETECTING RISK BEFORE THE FIRST SYMPTOMS: In the past decades, treatments for multiple sclerosis (MS) have advanced to the point where patients diagnosed today will experience few symptoms or relapses after their first attack. If we could identify individuals at high risk of developing the disease earlier, these therapies could halt the neurological damage that leads to the symptoms of this auto-immune condition.

That’s the goal of neurologist and researcher Dr. Adil Harroud at The Neuro. His lab found a group of proteins that were present in the blood of individuals who went on to develop MS up to 10 years before their first symptoms.

The next step will be designing sensitive and reliable tests to identify high-risk individuals earlier.

MULTIPLE SCLEROSIS - 8 NEW CLINICAL TRIALS! MS treatments have advanced by leaps and bounds in the last decade, but not every individual’s condition is as well managed as they'd like on current standard of care medications.

The MS Team at the Neuro CRU will be opening 8 new clinical trials in the coming months. In total, 18 studies will be underway this year, with options for RRMS, SPMS, PPMS, and our first trials for Clinically Isolated Syndrome (CIS) and Radiologically Isolated Syndrome (RIS).

The new approaches being studied hold the promise of better managing ongoing symptoms and relapses with fewer side effects.

SCLÉROSE EN PLAQUES – 8 NOUVEAUX ESSAIS CLINIQUES ! Les traitements pour la SP ont fait des progrès considérables au cours de la dernière décennie mais, pour certains patients, la maladie n’est pas toujours aussi bien prise en charge qu’on le souhaiterait avec la norme des soins.

L’équipe de la SP du Neuro CRU lancera 8 nouveaux essais cliniques dans les prochains mois. Au total, 18 études cliniques seront en cours cette année, incluant des essais pour les personnes atteintes de la SP cyclique, la SP progressive primaire et secondaire. Nous mèneront également nos tout premiers essais pour le syndrome cliniquement isolé (SCI) et le syndrome radiologiquement isolé (SRI).

Les nouvelles approches qui seront étudiées pourraient mieux gérer les symptômes et les rechutes, tout en comportant moins d’effets secondaires.

PROPELLING CLINICAL TRIALS FOR NOVEL THERAPEUTICS: Last week at , the manager of our Platform for Advanced Clinical Therapies (PACT), Theresa Degenhard presented a poster on how the PACT’s highly specialized consulting and trial management services can facilitate the translation of novel therapeutics into clinical studies.

The meeting of the American Society of Gene & Cell Therapy brought together industry, academia, regulatory stakeholders, non-profits and patient advocates to celebrate the strides made in providing clinical studies involving gene and cell therapies, as well as to identify critical next steps.

The Canadian Meet-Up underscored how vital it is for groups across the country to exchange knowledge and build collaborations. The PACT is excited to contribute to writing the future of novel treatments.

PROPULSER LES ESSAIS CLINIQUES POUR LES THÉRAPIES DE POINTE: La semaine dernière, lors du congrès , Theresa Degenhard, gestionnaire de notre Plateforme des thérapies cliniques avancées (PACT) a fait une présentation par affiche des services hautement spécialisés de consultation et de gestion d'essais cliniques de la PACT et comment ils peuvent faciliter la mise en œuvre de nouvelles thérapies dans le cadre d'études cliniques.

Le congrès de l'American Society of Gene & Cell Therapy a réuni des représentants de l'industrie, du milieu universitaire, des instances réglementaires, des organismes sans but lucratif et des associations de patients afin de célébrer les progrès dans la réalisation d'études cliniques sur les thérapies géniques et cellulaires, et de définir les prochaines étapes essentielles pour avancer.

Une rencontre des représentants d'institutions canadiennes a souligné l'importance d'échanger des connaissances et de développer des collaborations à la grandeur du pays. La PACT est fière de contribuer à façonner l'avenir des nouveaux traitements.

JOURNÉE INTERNATIONALE DES ESSAIS CLINIQUES ! Derrière chaque traitement approuvé, il y a une communauté de professionnels de la recherche qui permet aux patients d’accéder à de nouveaux traitements plus rapidement.

Au Neuro CRU, les coordinateurs de recherche clinique veillent à ce que les patients comprennent pleinement leurs droits ainsi que les risques et bénéfices de participer à un essai. Ils font le pont entre les patients et leur équipe soignante et peuvent obtenir des réponses rapides entre deux rendez-vous. Bien au-delà de leur rôle règlementaire et éthique, ils constituent un soutien essentiel et constant pour les patients et leurs proches confrontés à un pronostic difficile.

INTERNATIONAL CLINICAL TRIALS DAY: Behind every approved therapy, there is a community of research professionals who are helping to bring new treatments to patients more quickly.

At the Neuro CRU, clinical research coordinators educate patients, ensuring they fully understand their rights as well as the risks and benefits of participating in a trial. They act as a bridge between patients and their treating team, and can get answers patients need between appointments. Beyond the important regulatory and ethical role they fulfill, they are a crucial and consistent support to patients and their families who face a tough prognosis.

The Neuro is home to one of the largest neurological clinical research centres in Canada. Our clinical research coordinators are truly at the heart of research breakthroughs and help us lead the development of cutting-edge therapies.

SEMAINE DES SOINS INFIRMIERS ! Les infirmières et infirmiers de recherche en neurologie sont aux premières loges des avancées dans les traitements de pointe – un domaine passionnant et en constante évolution.

Ce sont les professionnels de la santé qui passeront le plus de temps avec les participants aux essais cliniques. Ils dispensent les traitements expérimentaux à l'étude, gèrent les perfusions et assistent les anesthésistes lors d’une infusion par voie intrathécale. Ils évaluent en continu le bien-être des participants lors de leurs visites au Neuro CRU.

En soutenant les patients et leurs proches lors de certains de leurs moments les plus difficiles, ils ont un impact majeur sur la qualité de vie des individus qui font face à des maladies neurologiques. Ils contribuent à faire avancer la recherche et nous ne pourrions pas faire ce que nous faisons sans eux !

NATIONAL NURSING WEEK! Research nurses have a front-row seat for the development of cutting-edge treatments for neurological diseases, an exciting and constantly changing field.

They are the healthcare professionals that will spend the most time with participants taking part in clinical trials. They administer the experimental therapies, manage IV infusions and assist anesthesiologists during intrathecal injections. They continuously assess the wellbeing of participants during their visits to the Neuro CRU.

By supporting patients and their loved ones during some of their most challenging moments, they have a major impact on the quality of life of those facing a neurological disease. They help to move research forward and we couldn't do what we do without them!

Le neurochirurgien de renom, Kevin Petrecca, MD, a fait équipe avec Frédéric Leblond, professeur de génie physique à Polytechnique Montréal pour développer un projet révolutionnaire : une sonde capable de mesurer les ondes lumineuses pour différencier les cellules anormales des cellules saines en bordure d’une tumeur cérébrale.

L’équipe de Découverte à Radio-Canada s’est entretenu avec les deux chercheurs pour mieux comprendre comment cet outil novateur pourrait réduire le risque de rechute d'un cancer en éliminant complètement les cellules anormales qu’on ne peut distinguer à l’œil nu.

D’ici 2030, 1 million de personnes au Canada seront atteintes de démence. Des traitements qui permettent de ralentir le cours de la maladie n’en sont qu’à leurs débuts, mais des essais cliniques en cours visent une nouvelle cible : la protéine tau, hautement reliée à la progression de la maladie.

L’équipe de Découverte à Radio-Canada s’est entretenu avec le Dr Simon Ducharme, neuropsychiatre et chercheur principal au Neuro CRU, à propos des traitements approuvés – et controversés – pour la maladie d’Alzheimer. ALZHEIMER'S DISEASE: NEW AVENUES FOR TREATMENT. By 2030, 1 million people in Canada will be living with dementia. Treatments that can slow the progression of the disease are still in their infancy, but new clinical trials are aiming at a new target: tau protein, which is strongly associated with the progression of the disease.

The team from Découverte at Radio-Canada spoke with Dr. Simon Ducharme, neuropsychiatrist and principal investigator at the Neuro CRU, about the approved—and controversial—treatments for Alzheimer's disease. They also discussed new avenues of research and the trials that are targeting tau.
