🚨 WE’RE HIRING – PART-TIME CARE COORDINATOR | DURHAM REGION 🚨

At The Core Home Health Inc. is looking for a motivated, organized and compassionate Part-Time Care Coordinator to join our growing team.

This is a great opportunity for someone who enjoys a combination of healthcare coordination, community outreach, relationship building and working independently in the community.

📍 Durham Region
🗓️ 3 days per week
🚗 2 community road days + 1 office day
📞 Every other weekend on-call

What you’ll be doing:

💙 Scheduling and care coordination
💙 Supporting communication with caregivers, clients and families
💙 Community outreach throughout Durham Region
💙 Building relationships with healthcare and community referral partners
💙 Assisting with new client consultations
💙 Researching and attending community events and outreach opportunities
💙 Supporting day-to-day office coordination

We’re looking for someone who is:

✔️ Professional, dependable and highly organized
✔️ An excellent communicator
✔️ Comfortable working independently
✔️ Confident meeting new people and representing our organization in the community
✔️ Experienced in healthcare, coordination, customer service, social services or a related field
✔️ Comfortable learning scheduling and care-management software

Driving Requirements

Because this position includes regular community road days throughout Durham Region, applicants must have:

🚘 A valid Ontario driver’s license
🚘 Reliable access to a vehicle
🚘 The ability to independently travel throughout Durham Region, including Pickering through Clarington
🚘 A current driver’s abstract will be required
