GSM. Ever heard of it? 🩺

1 in 3 women experience Genitourinary Syndrome of Menopause, yet most of us suffer in silence because we don’t even know it has a name.

Vaginal dryness, pain during s*x, urinary changes... all totally treatable. But here’s what nobody talks about: it’s killing your libido too.

When s*x is uncomfortable or painful, desire naturally drops. It’s not in your head. Your body is sending signals.
Think low libido is just stress or age? Think again. GSM might be part of the answer.

Swipe through to learn what’s really going on, then take our Low Libido Quiz (link in bio) to discover what’s affecting your desire. 💜

Your s*xuality matters. Your comfort matters. Let’s normalize these conversations.

Everyone talks about what estrogen does to your face. Almost nobody talks about what it does below the belt.

We accept it in the mirror — cheeks losing fullness, drooping under the eyes, crow’s feet. The same drop in estrogen is doing the same thing to tissue you can’t see: thinner, drier, a shift in pH, a changed local microbiome.

It has a clinical name — genitourinary syndrome of menopause, or GSM — and it affects nearly every postmenopausal woman to some degree. It often starts in perimenopause.

The part I want you to hear: this is one of the most treatable things I see. Daily moisturizers. Lubricants before intimacy. And where it’s appropriate, local estrogen alongside pelvic physiotherapy — which is genuinely effective.

But timing matters more than people realise. If you wait until intimacy already hurts, your nervous system starts to associate the two, and you’re then treating a pain loop as well as the tissue. Acting early is easier than reversing that.

If nobody has told you this before, you’re not the only one. Save it, and send it to a friend in the same decade.

If s*x has never quite followed the script you were taught, the problem was never you. It was the script.

Most of us learned one model of female s*xual response: excitement, plateau, climax, resolution. A straight line. One direction, one order, every single time. It was built by Masters & Johnson, and for decades it was the whole story.

Then Rosemary Basson described something different — a circular model. Desire is driven by intimacy and context, not just a spark that arrives out of nowhere. There’s no fixed starting point. Desire often follows arousal rather than leading it. And the outcome is flexible: satisfaction doesn’t have to look the same way twice.

So many women spend years quietly assuming they’re broken, when what actually happened is that they were handed the wrong map.

If any of this sounds like you, I’d love to hear it in the comments — and save this for the next time that thought creeps in.

We have been taught that libido should be driven by spontaneous desire.

But that is not the case for most women.

If you don’t have spontaneous desire it doesn’t mean you have low libido, it just means you need a setting to allow you to respond to get there.

Low libido in perimenopause and post menopause is a common condition we see in our patients at Sequence Wellness. It’s a condition that has many root causes, including hormones, stress and nervous system changes, fatigue and burnout, pelvic pain, dryness, relationship issues and even medication side effects.

The thing is, many patients don’t bring it up with their health care providers as it’s something they feel uncomfortable talking about.

We decided to break that barrier by creating our newest course: Low Libido in Perimenopause and Beyond. See our link in bio for more details

The DUTCH test is the cadillac of hormone testing. It tests for estrogens, progesterones, testosterone and other androgen, as well as DHEA and cortisol. I use this test a lot with my patients because it gives us the nuanced details of a woman's hormone imbalance, and with that precise information we can create a precise and root cause treatment plan to realign what's askew.
Another great thing about the DUTCH test is that samples are collected at home, so we can easily collect multiple samples in one day, giving us a really great picture of the patient's cortisol curve (stress response throughout the day).

This is a dutch test result that came in this week. The patient is exhibiting estrogen dominant symptoms, so even though it's not an extreme imbalance of estrogen: progesterone we want to improve her estrogen clearance via her CYP1A1 protective liver elimination pathway (not only for her current symptoms, but also for longterm health). We will also reduce the level of estrogen converting from her adrenal hormones by lowering the precursor hormone DHEA.

Naturopaths and herbalists love bitter herbs. At first they're awful, but soon you find you're craving them. Bitters prime the digestive system; like saying "Hey down below! Some food is coming soon, let's see what you can do". When we take bitters up to 60 mins before meals, it gives our stomach acid, pancreatic enzymes, bile and intestinal enzymes time to ramp up, and when the food finally arrives it is efficiently demolished as it should be.

Digestive bitters are recommended for conditions like SIBO, constipation, GERD and reflux, bloating, and other digestive concerns. But because it helps with nutrient absorption, I've also had patients swear by it for nail, skin and hair health too.

It's also a great way to counteract the impact of stress on the digestive function. Stress draws blood flow away from the GI tract (and therefore downregulates digestive enzyme secretion), whereas bitters do the opposite.

Many cultures have bitter (often alcoholic) beverages around their meals to improve digestion, and I would imagine that evolutionarily we were chewing on bitter twigs as we foraged for and prepared our meals. Our current setup of rushed eating in front of screens really puts our digestive process at a great disadvantage, so no wonder we have so many gut symptoms.

Be kind to your guts. Respect the (digestive) process. Drink bitters :)
