Nurse On Board Our Registered Nurses are advocates who help you make informed decisions about your health, while assisting you in navigating the health care system.

Our nurses, known as patient navigators and health care advocates, offer a range of services. They assist in communication among physicians and patients, helping clients to define health care goals, set agendas for doctor’s visits and shape expectations. In some cases, they help patients learn about options by researching diagnoses and treatments. Learn more about our services on our website.(https://www.nurseonboard.ca/our-services.html). We offer a free 30 minute, no obligation, phone consultation to see if our services are right for you. We'll follow up with a personal meeting to get you started. Afterwards you'll meet with your primary nurse to create your individualized health narrative. You'll benefit from our unique team approach to providing care, combined with a comprehensive electronic health record system. As our services are privately paid, some may be eligible for income tax deductions. Contact Us to discuss how Nurse On Board may be of assistance to you.

When should someone receive support with decision-making, and when should they continue making decisions independently?

Nurse On Board’s DecisionTracker™ is a structured monitoring service led by experienced Registered Nurses that helps identify changes in decision-making abilities while supporting personal autonomy.

The process focuses on five key areas: health care, mental health, housing, relationships, and finances. It helps identify concerns early and provides guidance for Substitute Decision-Makers (SDMs) on when additional support may be needed.

DecisionTracker™ helps balance independence with protection, but is not a formal legal or medical capacity assessment.

Preparing for the future isn’t only about paperwork - it can also be an act of care for the people you love. 🤍
One reason to prepare in advance is something Nurse On Board calls “Grief Brain.” Grief can affect both emotions and cognitive functioning, making it more difficult for loved ones to locate documents, manage legal matters, or learn unfamiliar tasks after a loss. Having important information organized ahead of time can help reduce some of that burden. Nurse On Board
