Include a team of health care professionals (i.e. physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians and others) who work together to provide health care for the community;
Ensure that people receive care they need in their communities by providing better care, closer to home;
Focus on chronic disease managem

ent, disease prevention and health promotion and work with other organizations in the local community such as Public Health Units and Community Care Access Centres.

📢 Labour Day Long Weekend Hours Reminder ☀️

As the Labour Day long weekend approaches, we'd like to remind our patients of our holiday hours.

📅 Nurse Practitioners will be closed on Friday and Monday.

🏥 The entire office will be closed on Monday, September 7.

✅ Doctors' offices remain open as scheduled on Friday.

🕒 After-hours services remain available on Saturday as usual.
