Why does treatment work? The honest answer is messier than most explanations admit.
We used to think improvement came down to what we did to the body. The bio-psycho-social model already shows us pain is more complex than tissue signals. Now we’re seeing that treatment response is just as layered.
Success depends on who delivered it. Whether the patient trusted them. Whether the explanation landed. Whether they felt heard. Whether patients believed treatment would help. All of it shapes the outcome, sometimes more than the technique itself.
The 'alignment' example illustrates this best. A patient gets told they are 'out of place', gets manipulated, feels better, and concludes they’ve been put back in place. The relief is real. The story behind it usually isn’t.
Good practice means listening and being honest that we don’t fully understand every mechanism at play. That serves patients better than confidently reaching for outdated/unproven stories about flushing toxins, breaking down fascia, and realigning pelvises.
Treatment success is rarely one thing. It’s a combination of factors filtered through a nervous system that responds to a great deal more than mechanical input alone.
