* Newcomers to Sarnia-Lambton who do not have a primary care clinician (NP or MD)
* Unattached persons who currently do not have on-going primary care from a NP or family doctor.

How do you apply?
*Stop in to the clinic (153 Christina Street S) and pick up a patient registration form
* Visit our website www.twinbridgesnplc.com and complete the online patient registration form (must be printed and brought in to clinic in person)

Twin Bridges NPLC is pleased to announce that we are part of the latest round of funding aimed at attaching all Ontarians to primary care. Our team will be growing as part of the funding announcement which will see an additional $2.88 million dollars invested into primary care teams across the Sarnia Lambton OHT.
