Dear Patients of Dr. Wareham-Fowler,
As many of you have heard by now, I am moving my practice to 97 Torbay Road, in the Renaissance Clinic Building. The name of the practice is Wareham-Fowler Family Practice.
Due to the transition to the new location and the creation of a new Electronic Medical Records system by Telus, the clinic will open Tuesday, April 7th.
However, our new Medical Office Administrator will be undergoing training by Telus on that day so phones will not be answered until Wednesday, April 8th, 2026.
In the meantime, you may send inquiries to our email address. This email address will start to be monitored on Friday, April 3rd, 2026:

All your medical records will be transferred over to the new electronic medical record. There is nothing for you to do. It will be all taken care of. Any appointments that you have already made will also transfer over.
 There is accessibility parking at the front of the building, and a couple other parking spaces as well. There is ample parking behind the building and a walkway to bring you to the front of the clinic.
I do appreciate that this may create some concerns or inconvenience for those who may need to see a healthcare provider during this time. However, during this transition, we will All need to exercise some patience.
If you require assessment by a Health Care Provider there are a number of options:
1. Walk-in clinics around the city such as 50 Mundy PondRd.,
2. You may contact 811,
3. Urgent Care Centre on Stavanger Drive;
4. Your Pharmacist
5. Of course for an emergency please go to the nearest emergency room or call 911.
I will be doing my best during this time to keep on top of incoming labs and investigations and will contact patients if needed.
I will also be creating a page over the next day or two it will be called “Wareham-Fowler Family Practice”
