Youth in Transition Worker Program The Youth-In-Transition Worker Program provides support to young people between the ages of 16-24 as they leave the care of the child protection system.

Thunder Bay Counselling Centre's Youth-In-Transition Worker Program supports young people between the ages of 16-24 as they leave the care of the child protection system, and become self-sufficient adults. The Program facilitates access to support services that help individuals build resilience as they exit care, and enhance their quality of life as they enter adulthood. Youth-In-Transition Worker

s work one-on-one with individuals to:

-Access health and mental health services
-Secure stable housing
-Obtain education and training information
-Connect to employment services
-Identify and access life skills support
-Locate legal services
-Determine and access additional support as needed
