Meet Amy, the Program Manager with Unison Health and Community Services and the program.

Amy's focus is on equitable attachment. She works to ensure our communities are receiving primary healthcare resources and services by leading the community planning of attaching members of our communities to local clinics and healthcare facilities.

She works directly with doctors, nurse practitioners, and organizations to provide resources directly to our community, and highlight areas to identify vulnerabilities in accessing healthcare.

Amy shares "So many people in our community do not have a family doctor. They need our program's support. Those with language or financial difficulties, those who face discrimination, they can go through us. We [Connect to Care] want to ensure every single person in North York and Northwest Toronto is attached to a family doctor and no one is left behind."

If you see Amy in the community, say hi!

Today marks Canada's 12th year in conclusion of the Truth and Reconciliation Commission.

Today, on National Truth and Reconciliation Day, otherwise known as Orange Shirt Day, we acknowledge the First Nations, Inuit, and Metis survivors of Canada's Residential School system and remember the children who did not return home to their families and the lives that were forever impacted.

Today is an opportunity to not only reflect on the past but how we can contribute positive change towards a better future for our communities. Connect to Care is committed to the health and wellness of those in our communities that face barriers due to factors like language, financial circumstances, and discrimination.

Together, we can work towards a healthier future for our communities.

We are so glad for the experience to meet new members of the community and discuss what we do.

Thank you to KMC Community for your help in welcoming Connect to Care and our work in the community!
