Together with Catholic Charities of the Archdiocese of Toronto, Synergy Toronto 2026 offers a three-day program filled with opportunities to learn, connect, share ideas and collaborate.

Through keynote presentations, workshops, panel discussions and interactive sessions, attendees will explore important topics including leadership, advocacy, housing, mental health, poverty reduction, reconciliation, synodality and service to those on the margins.

Through Let’s Chat, trained volunteers provide friendly, one-on-one conversations and social connection to people who may be experiencing loneliness or isolation.

Whether it’s sharing stories, talking about the day or simply having someone there to listen, our volunteers create moments of connection that can make a meaningful difference. 💬

At Saint Elizabeth Foundation, we’re proud to support programs like Let’s Chat that bring compassionate, person-centred support beyond traditional care and into the everyday lives of the people and communities we serve.

Last week, the Montréal International Palliative Care Congress at Palais des congrès de Montréal, brought together leaders, clinicians, researchers, advocates and communities from across Canada and around the world to learn, connect and reflect on the future of palliative and end-of-life care.

This year’s Congress also marked an important milestone: 50 years of palliative care in Canada. At Saint Elizabeth Foundation, we are proud to be part of that continued evolution, building on SE Health’s 118-year history of providing compassionate end-of-life care in the place patients and families call home.

Throughout the Congress, conversations explored many of the issues that continue to shape this work: equity, community, caregiving, grief, spirituality, education, lived experience and the importance of seeing the whole person behind the illness.

Our Executive Director, Fr. Dr. Matthew Durham, had the opportunity to connect with colleagues from across Canada and around the world and share more about the work of the Saint Elizabeth National Centre for Equity and Innovation in End-of-Life Care.

Creating more equitable and compassionate end-of-life care takes all of us— listening to communities, learning from one another, sharing what works and continuing to challenge what can be done better.

We’re grateful for the conversations, new connections and shared commitment that continue to move this important work forward.

This week, the Saint Elizabeth Foundation National Centre for Equity & Innovation in End-of-Life Care was proud to sponsor a Strategic Engagement Session hosted by Canada’s Pediatric Palliative Care Alliance | Alliance des soins palliatifs pédiatrique at the Hôpital de Montréal pour enfants | Montreal Children's Hospital.

The session brought together people with lived experience, clinicians, researchers, health leaders and policymakers from across Canada to contribute to the development of a National Action Plan for pediatric palliative care.

Through shared perspectives, expertise and meaningful discussion, participants explored how we can strengthen access, quality and sustainability in pediatric palliative care, with a shared goal of ensuring every seriously ill child and their family can access high-quality palliative care, when and where they need it.

We are proud to support this important national conversation and grateful to everyone contributing their knowledge, experiences and voices to help shape the future of pediatric palliative care in Canada.

Earlier this week, colleagues from across SE Health visited Journey Home Hospice Toronto with a thoughtful donation of new pyjama sets for those receiving care.

It was a simple gesture with a meaningful impact, and a wonderful reflection of the compassion and generosity that exists across the SE Health teams.

At Saint Elizabeth Foundation, we are grateful for the many ways our colleagues, community partners, donors and volunteers continue to show their support for Journey Home Hospice and the work happening every day to bring comfort, dignity and compassionate care to people at the end of life.

Thank you to our SE Health colleagues for taking the time to give back and for continuing to stand behind this important work.

This past Sunday was a wonderful day of community, connection and generosity at the McConkey Screech Open, held at Markham Green Golf Course! 🏌‍♂️ ⛳

A heartfelt thank you to Patrick and the entire McConkey family for hosting this special event in support of Journey Home Hospice, a program of Saint Elizabeth Foundation.

Community-led events like this are a powerful reminder of what can happen when people come together around a shared cause. To everyone who participated, sponsored, volunteered, donated or helped make the day possible — thank you.

Your generosity helps support the compassionate, dignified end-of-life care provided at Journey Home Hospice for people experiencing homelessness and housing insecurity.

At Saint Elizabeth Foundation | Journey Home Hospice, our kitchen volunteers help prepare warm meals while contributing to the welcoming environment that makes our hospice feel like a home.

Whether it’s preparing meals, helping behind the scenes, or simply giving their time, volunteers are an important part of the care and community that surrounds Journey Home Hospice.

Transforming end-of-life care takes collaboration.

The Saint Elizabeth National Centre for Equity and Innovation in End-of-Life Care works alongside organizations and healthcare professionals to advance more equitable, compassionate approaches to end-of-life care.

Grounded in evidence, experience, and proven care models, our team provides no-cost expert guidance and program support to help organizations build capacity, strengthen specialized services, explore new approaches, and improve access to care for equity-deserving populations.

Through consultation, community engagement, knowledge exchange, and advocacy, we are working together to address barriers to care and create meaningful, sustainable change.

Our work focuses on four priorities: improving access to care, advancing health equity, innovating specialized services, and building a sustainable future.
