Volunteers are at the heart of Saint Elizabeth Foundation | Journey Home Hospice.

Through their compassion, generosity, and willingness to give their time, volunteers help create meaningful moments of connection, comfort, and support for those we serve. Whether offering a listening ear, lending a helping hand, or sharing a simple act of kindness, their impact is felt every day.

We are grateful for the individuals who choose to give back to their communities and help make compassionate care possible.

For more than 118 years, Saint Elizabeth has been rooted in community care and supporting those most in need. Today, through the Saint Elizabeth Foundation, that legacy continues by advancing equitable, compassionate end-of-life care across Canada.

Through the annual Saint Elizabeth Walk, now open virtually, communities across the country come together through walks, runs, and rides to support local hospice and palliative care initiatives, Hike for Hospice events, and compassionate end-of-life care programming.

Open to everyone nationwide, the Walk is a meaningful way to stay active, give back, and be part of something larger.

Together, we walk in support of dignity, compassion, and care for all.

Volunteers at Saint Elizabeth Foundation are at the heart of what we do, and we’re excited to celebrate the launch of our new Friendly Visitors program supporting the SE Health Erie-St. Clair care clinics across Windsor & Essex County.

This amazing group is helping create warm, welcoming spaces through connection, kindness, positivity — and even special treats from the comfort cart — brightening the day for everyone they meet.

Thank you for bringing such enthusiasm and heart to this meaningful new volunteer initiative. You remind us that small moments of human connection can make a big difference for our patients and families. ❤️

Yesterday, Saint Elizabeth Foundation | Journey Home Hospice Executive Director Fr. Dr. Matthew Durham, and Cindy (Cynthia) M. Dwyer, MA. BCC virtually represented Saint Elizabeth Foundation in the 79th World Health Assembly working session of the Interfaith Advocacy Network for Palliative Care in Geneva, Switzerland.

We are grateful to the International Association for Hospice and Palliative Care (IAHPC) for the invitation to contribute to this important global conversation bringing together faith leaders, healthcare professionals, and advocacy partners committed to advancing equitable access to palliative care worldwide.

The session created an opportunity to share the work being done across Canada to support individuals with housing insecurities, while also learning from global partners dedicated to compassionate, faith-informed models of care.

Through the Saint Elizabeth Foundation National Centre for Equity and Innovation in End-of-Life Care, we are helping convene what we often call a “coalition of the willing,” bringing together partners across healthcare, hospice, academia, philanthropy, and communities to advance more equitable, compassionate, and innovative end-of-life care for all.

Because every life matters, to the very last moment.

Recently, Saint Elizabeth Foundation | Journey Home Hospice Executive Director Fr. Dr. Matthew Durham spoke with The Catholic Register following his presentation at St. Jerome's University on the evolution and impact of Journey Home Hospice.

The story highlights the growth of our specialized hospice model supporting individuals experiencing homelessness and housing insecurities – from the opening of our initial four-bed hospice in Toronto, to the expansion of services in Windsor, and the recent launch of the National Centre for Equity and Innovation in End-of-Life Care.

Grounded in dignity, compassion, and equity, the article also explores how Journey Home Hospice is helping shape more inclusive approaches to hospice palliative care across Canada through education, collaboration, and advocacy.

We are grateful to be featured in this month’s Catholic Charities of the Archdiocese of Toronto newsletter in recognition of National Hospice Palliative Care Week.

The article highlights how Saint Elizabeth Foundation | Journey Home Hospice continues to advance compassionate, equitable end-of-life care for individuals experiencing homelessness, marginalization, and social isolation through initiatives including Journey Home Hospice, the National Centre for Equity & Innovation in End-of-Life Care, and our Spiritual Health, Grief & Bereavement programs.

As this year’s theme, Palliative Care Everywhere, reminds us, compassionate care belongs in every community, for every person. We are grateful to our staff, volunteers, supporters, and community partners whose dedication helps make this work possible each day.
