As HPCO 2026 has come to a close, we’re reflecting on a truly inspiring few days of learning, collaboration, and connection.

Saint Elizabeth Foundation was proud to serve as the Presenting Sponsor of this year’s Hospice Palliative Care Ontario Conference, supporting an event that brought together professionals, researchers, volunteers, advocates, and leaders from across the hospice palliative care sector to share knowledge, explore innovative approaches, and advance compassionate care for all.

Throughout the conference, our team was honoured to connect with colleagues and partners, participate in meaningful conversations, and contribute to important discussions on equity, harm reduction, and improving access to hospice palliative care for marginalized populations. We also enjoyed celebrating and networking with attendees at various social engagements throughout the conference, where new connections were formed and ideas continued to flourish beyond the session rooms.

The conference concluded with a collaborative research roundtable, bringing professionals from across the palliative care space together for shared learning and thoughtful discussions about shaping the future of hospice and palliative care research in Canada.

Thank you to Hospice Palliative Care Ontario , the speakers, organizers, sponsors, and attendees who made this year's conference such a meaningful experience. Congratulations to Saint Elizabeth Foundation volunteer Nancy McElhinney, on receiving the June Callwood Award at this year's Hospice Palliative Care Ontario Conference.

Established in 1994, the June Callwood Award recognizes outstanding hospice volunteers across Ontario whose compassion, dedication, and service make a meaningful difference in the lives of others.

Nancy's kindness, generosity, and commitment to supporting Journey Home Hospice residents and their loved ones embody the spirit of this award. We are incredibly grateful for all that she contributes to our community and are proud to see her receive this well-deserved recognition.

Yesterday at the HPCO Conference, Saint Elizabeth Foundation | Journey Home Hospice was proud to contribute to important conversations on equity, innovation, and access in hospice palliative care through two sessions led by Medical Director Dr. Felicia Kontopidis and Executive Director Fr. Dr. Matthew Durham.

The sessions, Advancing Specialized Hospice Palliative Care for Structurally Vulnerable Populations: Practice, Policy, and System Innovation and Hack the System: Advancing Harm Reduction and Equity in Palliative Care, explored practical approaches to addressing barriers to care for individuals experiencing housing insecurity, poverty, substance use, mental health challenges, and other forms of marginalization.

Drawing on the innovative work of Journey Home Hospice, participants engaged in discussions around practice, policy, system innovation, and harm reduction, while exploring ways to strengthen the FOUNDATION of equitable, accessible, and compassionate end-of-life care.

Thank you to everyone who attended, shared their insights, and contributed to these meaningful conversations. Advancing health equity requires collaboration, creativity, and a commitment to ensuring that everyone has access to quality palliative care, regardless of their circumstances.

Proud to have our Journey Home Hospice leadership team at this year's Hospice Palliative Care Ontario Conference!

What an inspiring plenary session to start the conference. We're excited for three days of learning, collaborating, and networking.

Saint Elizabeth Foundation is honoured to be the Presenting Sponsor of this year’s Hospice Palliative Care Ontario Conference.

It was a privilege for our Executive Director, Fr. Dr. Matthew Durham, to open the conference and introduce the keynote speaker, Dr. Jared Rubenstein helping kick off what promises to be an inspiring and energizing gathering of hospice and palliative care leaders from across Ontario.

The energy in the room was incredible, reflecting the passion, innovation, and commitment that continues to drive excellence in hospice palliative care.

At Journey Home Hospice volunteers help create a welcoming and compassionate environment where residents feel seen, valued, and cared for. Whether through a conversation, a shared laugh, or simply being present, volunteers play an important role in helping ensure no one faces their end-of-life journey alone.

Through the annual Saint Elizabeth Walk, now open virtually, communities across the country come together through walks, runs, and rides to support local hospice and palliative care initiatives, Hike for Hospice events, and compassionate end-of-life care programming.

Open to everyone nationwide, the Walk is a meaningful way to stay active, give back, and be part of something larger.
