✨ Meet Team Grace: Niyat K.

We're proud to introduce Niyat K., X-Ray Technologist in our Outpatient Clinic, who has been part of the Toronto Grace Health Centre family for the past six months.

Niyat enjoys connecting with patients and colleagues while completing portable X-ray runs throughout the hospital. What stands out most is the supportive environment and the opportunity to work alongside dedicated healthcare professionals every day.

Toronto Grace Health Centre (TGHC) proudly presented its quality improvement initiative, "Implementing a Delirium Assessment Tool: Improving Delirium Screening Rates and Patient Safety," at the Sigma International Nursing Research Congress 2026 in Toronto.

This achievement reflects the commitment and collaboration of TGHC's Interprofessional Practice and Education team in advancing evidence-based practice and strengthening person-centred care. Implementing the Confusion Assessment Method Severity-Score tool has improved delirium screening rates, patient safety, and quality of care.

Thank you to everyone who contributed to this meaningful work and continues to champion excellence in clinical practice.
