Gabbing at book club about your hormone therapy (or lack there of) can cause an all out hormone therapy crisis where you’re left feeling confused and concerned. It needs to be said: we all are snowflakes peeps. How the hormone hits you is how it hits YOU. Everyone has different needs and responses. It is important to realize and accept how very vary, it actually is. Makes life easier when we understand. ✌🏻🫶🦋🩺

You may notice it when you get out of bed: stiffer feet, achy hips, slower recovery, or a body that suddenly feels less predictable.

I hear you ladies.

It’s easy to assume this is “just aging.” But in perimenopause and menopause, changing estrogen levels can affect your joints, muscles, sleep, and pain sensitivity.

Hormone therapy may be part of the picture (for some, not everyone), but they’re rarely the whole picture. Stress, sleep, strength, blood sugar, thyroid health, and previous injuries can all contribute.

Your body isn’t failing you. It’s giving you information about what may need support.

Progesterone is often described as calming, especially when it comes to sleep. But that is not every woman’s experience.
Some women feel more anxious, irritable, flat, foggy, or overwhelmed.

That does not mean they are imagining it, and it does not mean hormone therapy has “failed.” It means the response deserves a closer look.

The formulation, dose, timing, symptom pattern, mood history, sleep, stress, and the rest of the hormone plan all matter. Your symptoms are information, and good care should use that information to guide a more personalized strategy.

Hormone therapy should fit the woman, not force the woman to fit the protocol. This is why personalized care matters ladies.

So many women spend months, sometimes years, trying to understand why they no longer feel like themselves.

They’re told it’s stress, poor sleep, or simply “getting older.” Sometimes those things are part of the picture. But often, perimenopause is also in the picture.

Because symptoms can be subtle, inconsistent, and easily dismissed, many women are left connecting the dots on their own.

You are not imagining it, and you are not alone.

This is not to blame any one person - but as a society, and especially as healthcare professionals, we need to do better.

If you’ve tried vaginal estrogen and still have dryness, irritation, pain with s*x, urinary symptoms, or recurrent UTIs, please don’t assume it “didn’t work.”

The plan may simply be incomplete.

This is something many women are not told: you do not always have to pick just one vaginal estrogen product.

Some symptoms are primarily internal: vaginal dryness, pain with s*x, urinary urgency, recurrent UTIs.

Some symptoms are external: vulvar dryness, burning, irritation, or discomfort with wiping, clothing, intimacy, or movement.

And when symptoms are happening in more than one place, one product may not address the whole picture.
For some women, an internal option such as a vaginal insert, tablet, or suppository may be useful. For others, a cream may be helpful externally. And for many women, a combination approach is what finally makes the difference.

This is not about using more for the sake of more.

It is about using the right tool in the right place.

Genitourinary symptoms of menopause are common, real, and often undertreated. Dose, frequency, placement, consistency, product type, and symptom location all matter.

This is where individualized care makes all the difference ladies.

Joint pain, muscle aches, stiffness, tendon pain, slower recovery…these are some of the symptoms I hear about all the time in perimenopause.

And yet, they’re often dismissed as “just aging.”

A recent systematic review and meta-analysis of over 93,000 women found that perimenopausal women had a significantly higher risk of reporting muscle and joint symptoms compared with premenopausal women.

So yes, the menopausal transition matters.

Estrogen receptors are found in joint and connective tissues, and as estrogen shifts, it may influence inflammation, collagen production, tendon and ligament elasticity, muscle function, and recovery.

That does not mean every ache is hormonal.

It means we need to stop separating musculoskeletal symptoms from the broader menopause conversation.
If your joints, muscles, or tendons feel different than they used to, don’t just push through and hope it passes.

Midlife pain deserves context.
And it deserves better care than “it’s just your age.”

I have exposed myself. Utterly. I am sorry if I let you down.

But for real, how do people manage this without force feeding, and how can that be …healthy?

On a serious note - if in midlife it is actually important to at least try to hit those targets (90-100g/ day protein, 25g / fiber). The reality is that myself and my patients may not cross the finish line every single day, but we are close and that counts for a lot, over the long term.

And in truth, I often make it with one or the other, or just come generally close and I am actually rather dazzled with myself because left to do what I want….. mmmm…NOPE.

So why do we care? Oh lord well that’s a book, but suffice to say protecting your gut microbiome, metabolism, energy, mood, muscle mass, sleep, blood sugar, weight, I mean, I could go on but I won’t.

If we can get over the heinous glasses, you do need to try for that ladies. It will help you, in spades.

Keeping it real here folks.

One of the healthiest things a woman can protect in midlife is her capacity for wonder.

Not just her productivity.
Not just her image.
Not just her ability to hold everything together.

Wonder.

The part of you that still notices light moving across the floor, rain in the air, pine needles warming in the sun, or the moment your body finally exhales.

The world asks women to become practical, efficient, realistic, and endlessly available.
But becoming an adult woman should not require becoming hardened.

There is nothing silly about staying open to beauty.
There is nothing indulgent about awe.
There is nothing weak about remaining hopeful when life has made you tired.

Medicine comes in many forms.

Sometimes it is hormones, labs, nutrients, and treatment plans.

And sometimes it is remembering that you are still part of a living world.
