SHN is committed to improving access to healthcare services for our community. Our IPCT Clinic provides support for those who do not have a family doctor or other primary care provider, especially those with ongoing healthcare concerns.

The IPCT Clinic brings together several healthcare professionals, including Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Allied Health Professionals, and Patient Navigators and Ambassadors. Our team works together to provide coordinated support and management of your healthcare needs.
