Not every man with prostate cancer is a candidate for focal therapy.

HIFU and NanoKnife are designed for localized, well-defined cancer that can be mapped and treated precisely. When disease is multifocal, high-volume, difficult to localize, or extends beyond the prostate, surgery or radiation may be the more appropriate option.

Careful patient selection is one of the most important parts of focal treatment.

There are two approaches to prostate biopsy: transre**al and transperineal.

With a transre**al biopsy, the needle passes through the re**al wall before entering the prostate. With a transperineal biopsy, the needle passes directly through the skin between the sc***um and re**um into the prostate.

At UIC, we perform only transperineal prostate biopsies because this approach has been associated with lower rates of infection and bleeding compared with traditional transre**al biopsy.

For us, prostate cancer care means thinking about precision and quality of life at every step — starting with the biopsy.

Like many of our patients, Terry travelled from Calgary to UIC in Toronto for his NanoKnife (IRE) treatment of localized prostate cancer.

Five months later, he shared these follow-up results:

• PSA decreased from 15.5 to 1.8
• Follow-up MRI showed that the lesions were gone
• No urinary incontinence
• No erectile dysfunction
