Most women walk into a perimenopause appointment asking for one thing: check my hormones.

This obviously makes sense — but it can miss the bigger picture. Thyroid dysfunction, iron deficiency, and blood sugar imbalance can all produce fatigue, brain fog, and mood shifts that look nearly identical to a hormonal shift. Without ruling those out first, it's easy to spend months chasing the wrong explanation.

A full panel doesn't replace hormone testing — it puts it in context. Once the full picture is clear, a hormonal approach can be layered on if it's actually needed.

I get it. But in my practice, hormones are rarely the only thing worth ruling out.

Thyroid. Iron. Blood sugar. Inflammation markers. Many of these can produce symptoms that look identical to a hormonal shift — fatigue, mood changes, brain fog, sleep disruption.

A full panel tells you what’s actually driving what you’re feeling. Then, if hormones are part of the picture, that’s where a hormonal approach comes in — not before.

Anxiety. Sleep that won't hold. Fog that won't lift.

The explanation almost always stops at hormones.

But for a lot of women, the labs come back with "normal" hormones — and the symptoms stay exactly the same.

Sometimes it's not just hormones. One example is iron levels. And we can check iron levels by testing your Ferritin. Ferritin can drop during this transition, and low iron produces nearly the same symptom set as shifting hormones.

The pattern isn't always hormonal. Sometimes two systems are moving at once.

Estrogen doesn't just get produced. It has to be cleared, too. And when progesterone starts to decline and the balance between progesterone and estrogen becomes more erratic we need to help the bodies natural clearing pathways.

Most conversations stop at hormone levels — production, decline, fluctuation. Clearance rarely comes up at all.

Here's the piece that's often missing: once estrogen is metabolized, it's packaged for removal through the gut. But certain gut bacteria can "unpack" it before it leaves the body, sending some of it back into circulation instead of out. Fiber may help prevent that — binding to those metabolites in the digestive tract so more of them are cleared rather than reabsorbed.

This week, notice how much fiber actually lands on your plate in a given day — beans, vegetables, whole grains. Not to overhaul anything. Just to see the baseline.

Not because the care was wrong. Because perimenopause doesn't hold its patterns until your next scheduled visit. Estrogen shifts week to week. Progesterone responds to stress in real time.
