**The documentation that saves you during survey isn't usually written the day before survey... it's written every single day.**
One of the biggest mistakes I see long term care dietitians make is focusing only on clinical recommendations while overlooking **the story their documentation tells.**
Surveyors aren't just looking for *what* you recommended, they're looking for evidence that you:
βοΈ Identified the nutrition problem.
βοΈ Developed an appropriate intervention.
βοΈ Followed up and documented
βοΈ Communicated with the interdisciplinary team.
Strong documentation doesn't have to be long.
It has to be **clear, consistent, and show your clinical reasoning.**
