Not All Pain Is the Same

Many people think pain always comes from a muscle or joint—but that’s not always the case. Current evidence can explain why pain can come from different tissues in the body.

Here are three common types of pain I often assess in treatment:

1. Muscle & Joint Pain (Musculoskeletal)

This is the most familiar type of pain.
Examples:
• Tight shoulders from desk work
• Stiff necks
• Sore backs after lifting or training

This pain often responds well to: • Massage therapy
• Movement and mobility work
• Load management

2. Nerve-Related Pain (Neurogenic Pain)

Sometimes pain comes from irritation or sensitivity in the nervous system rather than the muscle itself.

Signs it might involve a nerve:

• Burning or sharp pain
• Tingling or numbness
• Pain that travels down an arm or leg
• Symptoms that change with certain movements or positions

This is where concepts from neurodynamics research (like Shacklock’s work) help clinicians understand how nerves move and respond to stress in the body.

3. Inflammatory or Tissue Irritation

Why injured tissue can become stiff or “contracted”

When your body heals an injury, it sends signals to repair the area. One important signal is TGF-β1, a chemical messenger that helps coordinate healing.

Here’s the simple version of what happens:

Step 1: Repair mode turns on
TGF-β1 tells certain cells (fibroblasts) to become myofibroblasts — special repair cells that can actually pull tissue together to close wounds.

Step 2: Tissue tightens
These repair cells create tiny contractile fibers (similar to muscle fibers) and pull on the surrounding tissue to stabilize the area.

Step 3: The feedback loop
As these cells contract, they release more TGF-β1, which signals even more contraction and tissue repair.

When healing goes too far
If this process continues for too long, it can lead to:
• Increased tissue stiffness
• Scar tissue buildup
• Reduced mobility
• Conditions like frozen shoulder or contractures

Ever wonder why your foot becomes more rigid when you push off while walking or running?

That’s thanks to the Windlass Mechanism.

When your big toe extends (bends upward) during the push-off phase of gait, it tightens the Plantar Fascia along the bottom of the foot.

This tightening raises the arch and stabilizes the foot, turning it from a flexible shock absorber into a rigid lever for propulsion.

Why this matters: • Efficient running and walking mechanics

• Arch support and foot stability
• Reduced strain on the foot and lower leg
If the windlass mechanism isn’t working well, it can contribute to issues like: • Plantar Fasciitis
• Arch pain
• Achilles or calf tightness

Massage therapy can help by improving mobility in the foot, calf, and big toe so this mechanism can function properly.

Can tight hamstrings be a symptom to intrinsic core function? See how hamstring tightness and tension can be relieved by performing several exercises (without actually manually treating the hamstrings!).

Using the pre-treatment assessment comparison for hamstring length, there is certainly an improvement after 10 minutes of working through intra-abdominal pressure, breathing, and scapular stability (without even stretching the hamstrings or hips in Megan's case).

This approach can be applied to other areas of symptoms, not just the hamstrings.
