👋 How can we draw reliable treatment conclusions when randomised controlled trials in orthopaedics aren't feasible?

👩‍⚕️ 👨‍⚕️ Conducting RCTs for surgical interventions comes with major ethical and practical hurdles, such as blinding limitations and strong patient preferences. That's where modern causal inference methods step in!

📰 A new scoping review in EFORT Open Review mapped 85 orthopaedic studies to analyse how clinicians and researchers use observational data to estimate causal effects.

🌟 Key Highlights:

👉 Top Methods: Propensity score matching (PSM) and inverse probability of treatment weighting (IPTW) are the most widely applied tools in orthopaedics.

❌ The Missing Step: Advanced frameworks like Target Trial Emulation (TTE) and Directed Acyclic Graphs (DAGs) are still heavily underutilised, despite their ability to eliminate fundamental study design errors.

📑 Protocol Transparency: Less than 6% of the included observational studies published a pre-registered protocol.

👁️‍🗨️ Expert Recommendation: Moving forward, incorporating causal diagrams (DAGs) and pre-registered protocols will greatly enhance the credibility of non-randomised orthopaedic research.

👉 Not necessarily! Objective laboratory assessments show that many patients continue to walk with subtle asymmetries, reduced hip extension, and lower joint power generation even long after surgery.

📰 A PRISMA-guided systematic review in EFORT Open Review analysed 50 studies evaluating in vivo biomechanics after THA to identify what truly drives functional recovery.

🔸 Surgical Approach Is Secondary: Long-term gait normalisation is barely influenced by whether a posterior, lateral, or direct anterior approach is used.

🔸 Anatomy Is King: Reconstructing femoral offset, anteversion, and joint geometry is crucial for restoring joint kinetics and muscle leverage.

🔸 Static X-rays Don't Tell the Whole Story: Dynamic dual-fluoroscopy imaging proves that "well-aligned" cups on static X-rays can experience dynamic edge-loading during daily movement.

👋 👩‍⚕️ 👨‍⚕️ Can free tissue transfer reliably treat massive soft tissue defects in knee periprosthetic joint infections (PJI)?

👉 When local rotational flaps are not feasible, free flaps offer a key reconstructive option for extensive knee defects. A systematic review and meta-analysis in EFORT Open Review analysed data across 14 studies and 185 patients to evaluate their success and safety.

🌟 Key Highlights:

📈 93% Survival Rate: Free flaps showed a pooled survival rate of 93%, demonstrating high reliability for complex limb reconstruction.

🦵 Limb Salvage: Uncontrolled infection led to limb amputation in only 8% of cases, while the reinfection or persistent infection rate was 16%.

🔩 Most Common Flaps: The free latissimus dorsi and anterolateral thigh (ALT) flaps were the most performed reconstructive solutions.

✔️ Technique Matters: Flap failure was mostly driven by technical errors and microvascular complications rather than intrinsic flap issues, underscoring the need for specialized orthoplastic skill.

🌟 To address this, the Innovative Health Initiative (IHI) has launched MedTechLabel, a collaborative project developing a universal digital labelling framework for medical devices and in vitro diagnostic tools across all classes.

👉 Scanning a QR code will give healthcare professionals and patients immediate access to up-to-date, targeted product information in their choice of language. This initiative reduces physical packaging waste, minimizes administrative burdens on manufacturers, and improves clinical accessibility.
