It is a pleasure to welcome Dr. Mamta Mamik from NY/USA to Zurich for a two-week immersion in Neuropelveology.
This week, we operated together on a male patient with a severe femoral nerve neuropathy caused by a mesh after inguinal hernia repair. After years of compression and axonal damage, we performed a decompression and implanted a Medtronic neurostimulator to promote axon regeneration through low-frequency antidromic stimulation, combined with a program for quadriceps muscle training.
These cases show clearly what Neuropelveology truly is: not mass medicine, not routine, not standardized algorithms - but intelligent, individualized, high-level medical reasoning. Every case is unique. In Neuropelveology, every diagnosis is earned, never given - and the success of every treatment depends on the exactness of this diagnosis. Neuropelveology is a clinical discipline built on listening to the patient. The examination of the pelvic nerves is essential, while all diagnostic tools, including imaging, remain secondary. As Jón Einarsson /Harvard Uinversity once said by assisting me in Zurich, diagnosis in this field is “detective work”: a patient and time intensive, meticulous process of synthesizing a multitude of seemingly confusing elements and reorganizing them through the lens of pelvic neuroanatomy. Only then does a clear diagnosis emerge.
Neuropelveology goes far beyond gynecology. It belongs equally to women, men, and even children. And although the field was born 20 years ago from my pioneering work in gynecology, more and more neurosurgeons and urologists are entering the discipline.
Neuropelveology is not a collateral subspecialty in Gynecology, it is a medical discipline of its own, and its future lies in bioelectronic medicine and pelvic neuromodulation.
