How should we interpret clinical trial data in melanoma?

Prof. Paolo Ascierto highlights an essential point: clinical trials generate crucial evidence, but they are conducted in highly selected patient populations. Strict inclusion criteria mean that some patients we routinely see in clinical practice, such as those with brain metastases, leptomeningeal disease or poorer performance status, may be underrepresented or excluded.

For this reason, trial results should never be viewed as simply “positive” or “negative.” They need to be interpreted in the context of the study design, inclusion criteria, assumptions, strengths and limitations.

Looking beyond the headline result, and critically examining what the data can and cannot tell us, is key to translating evidence into real-world clinical practice.

🧬 Advancing CAR T-Cell Therapy in Cutaneous Malignancies 🧬
Prof. Dr. Ugur Uslu (University of Würzburg, Germany) discusses the current landscape and future directions of CAR T-Cell therapy for skin cancer.

While CAR T-cell therapy has demonstrated remarkable efficacy in hematological malignancies, its application in solid tumors remains a critical research frontier.

Prof. Dr. Uslu highlights that while current treatment options for advanced melanoma are promising, approximately half of these patients either do not respond or eventually experience a long-term relapse.

To address this unmet clinical need, his laboratory is actively developing new CAR constructs specific for antigens against melanoma and Merkel cell carcinoma.

By exploring combination therapies—such as combining CAR T-cells with checkpoint blockade or oncolytic viruses—and engineering cells to secrete cytokines or antibodies, the goal is to optimize efficacy and rapidly transition these advancements into early-phase clinical trials.

From vision to reality.✨💪
The first EADO Skin Cancer Center of Excellence Preceptorship at Hospital Clínic Barcelona successfully brought together 25 outstanding young dermato-oncologists from 9 European countries for an immersive day of learning, collaboration, and mentorship.

Participants gained first-hand insights into the organization of a leading Skin Cancer Center, translational research, clinical innovation, digital transformation, and the growing role of artificial intelligence in dermato-oncology.

Beyond the scientific program, the Preceptorship created a unique environment for exchanging ideas, building connections, and inspiring the next generation of clinician-scientists who will shape the future of skin cancer care across Europe.

This is only the beginning.🚀

Did you know that there is a new and different classification for Lentigo Maligna?
At the recently concluded 22nd EADO Congress in Prague, Prof. Dr. Iris Zalaudek (), Chair of the Department of Dermatology and Sexually Transmitted Diseases at the University of Trieste (), shared groundbreaking insights from their latest research on managing this type of facial melanoma.

💡 Two Distinct Types of Lentigo Maligna

▫️Easy to Treat: Often presents as a single spot with well-defined borders. It can be easily surgically removed, and the risk of recurrence is considerably low. The team proposes standard surgical and safety margins for these cases.

▫️Difficult to Treat: These are usually very large, multi-focal tumors with ill-defined borders. Simple surgical excision carries a very high risk of involved margins and an increased risk of recurrence. For these, the team proposes adjuvant Imiquimod treatment—treating it as a field therapy since these cases may originate from field melanomization.
