15 years as a plastic surgeon have taught me that the best results are rarely about doing more.

I’ve also learned to let go of perfection. Every face is different. Perfect symmetry isn’t the goal. Preserving what makes you recognisable while restoring balance is.

Fat or filler: which is the better choice for facial volume loss?

Filler can be an effective option for smaller, targeted areas. It is quick to inject, adaptable and can create subtle structural support. But when larger volumes are needed, repeatedly adding filler can begin to distort natural proportions and create an overfilled appearance.

Fat transfer offers a different approach. Using your own tissue, it can restore volume more broadly while supporting the natural structure of the face, with a proportion of the transferred fat remaining long term.

Ultimately, it is not about which is better. It is about choosing the right approach for your anatomy, degree of volume loss and desired outcome.

“Ozempic face” isn’t simply about losing weight. It’s about what happens to the face when volume is lost rapidly.

With significant weight loss, we can see a reduction in facial fat that previously provided volume and structural support. As this volume decreases, areas of the face may begin to appear more hollow, while skin laxity and sagging can become more noticeable.

But not every case of “Ozempic face” looks the same. The degree of change depends on factors such as the amount and speed of weight loss, age, skin quality, facial anatomy and where volume has been lost.

Understanding what has changed is therefore the first step. Is it primarily volume loss? Skin laxity? A change in facial support? Or, as is often the case, a combination?

Different problems require different solutions.

The HIT Lift®️ (Hidden Incision Temple Lift) was created from a simple observation: in younger women and men, early facial ageing is not always a problem of excess skin. Often, it is the deeper tissues that have begun to shift from their original position.

In these cases, removing skin is not necessarily the answer.

Can you have a facelift without removing skin? Yes.

With the HIT Lift®️, the goal is not to pull or remove skin. It is to reposition the deeper anatomical layers of the face.

Through a hidden incision approximately 2.5 cm behind the hairline, the deeper tissues are repositioned while the skin remains largely untouched. This helps create a naturally refreshed result without the pulled appearance traditionally associated with facelifts.

Leaving some skin behind is intentional. As healing progresses, gravity naturally places tension on the tissues. Avoiding unnecessary tension on the incision helps support a finer, more discreet scar that remains hidden within the hairline.

The neck is different.

With the PR Neck Lift™️, excess skin may need to be removed. In this case, incisions are carefully positioned within the natural contours of the ear, including the tragus and behind the ear crease, to keep them as discreet as possible.

The principle remains the same: reposition what needs to be repositioned, remove only what needs to be removed, and leave as little visible evidence of surgery as possible.

From Thailand to Zurich for a result designed to last.

Our patient travelled from Bangkok to AUMAR for a highly personalised surgical journey combining the HIT LIFT® Extended with platysmaplasty and fat transfer.

The aim was not to change his face, but to restore structure where it had been lost, redefine the face and neck, and selectively restore volume while preserving the features that make him recognisably himself.

These images show an early stage of recovery, just four days after surgery. Swelling and healing are still ongoing, and the final result will continue to evolve over the coming weeks and months.
