FFN - A Global Network to Improve Fragility Fracture Management and Prevention The Fragility Fracture Network (FFN) is a global network of activists.

The mission of the Fragility Fracture Network is to promote globally the optimal multidisciplinary management of the patients with a fragility fracture, including secondary prevention

The FFN focuses activity on 6 key themes:

• Peri-operative care
Improving the peri-operative care of fragility fracture sufferers has, and will continue to be, a major focus for FFN members and a theme of FFN Congresses . The 2 key components of peri-operative care are anaesthesiology and orthopaedic-geriatric co-care (otherwise known as orthogeriatrics).

• Surgical treatment
The primary challenge facing surgeons is how to obtain secure fixation of an implant to osteoporotic bone. Leading initiatives from the AO Foundation (www.aofoundation.org) and Osteosynthesis and Trauma Care Foundation (www.otcfoundation.org) have identified and disseminated best practice.

• Rehabilitation
Returning hip fracture sufferers to their pre-fracture level of function and independence is a major challenge. In this regard, provision of effective rehabilitation is a vital component of any system of care for fragility fracture sufferers.

• Secondary prevention
Effective secondary prevention must urgently be established as a standard part of post-fracture care throughout the world. The International Osteoporosis Foundation Capture the Fracture Campaign (www.capturethefracture.org) provides extensive resources for those seeking to eliminate the secondary prevention care gap.

• Research and education
FFN Special Interest Groups are active in establishing a minimum dataset for hip fracture audit, developing evidence-based pathways for vertebral fracture patients and defining key questions in rehabilitation research.

• Changing healthcare policy
Driving policy change that will raise fragility fracture care up the healthcare agenda across the world is a major aim of the FFN. FFN Congresses and the website provide a platform for sharing success in the policy arena. 2026 NICE Osteoporosis Risk Assessment Guideline Update
The National Institute for Health and Care Excellence (NICE) released an update to its 2012 guideline ‘Osteoporosis: Assessing the Risk of Fragility Fracture’. The new 2026 guideline ‘Osteoporosis: Risk Assessment’ covers identifying adults for fragility fracture risk assessment, risk assessment methods, identifying vertebral fragility fractures and treatment criteria.

Global Perspectives on the Care Gap in Fragility Fracture Management

Fragility fracture care remains suboptimal worldwide despite strong evidence supporting timely surgery, orthogeriatric care, early mobilization, unrestricted weight-bearing, and secondary fracture prevention.

International quality standards and registry-based audits have translated this evidence into measurable indicators that improve care in many high-income health-care systems.

Implementation in low- and middle-income countries (LMICs) is limited primarily by health-system constraints, including workforce shortages, delayed presentation, limited infrastructure, and fragmented care pathways, rather than by lack of evidence.

Emerging initiatives demonstrate that evidence-based quality standards can be successfully adapted to local contexts while preserving their core clinical principles.

Orthopaedic surgeons have a central role in leading multidisciplinary care, promoting secondary fracture prevention, and driving quality improvement across the entire fragility fracture pathway.

Future progress depends on strengthening health systems, embedding auditing and certification programs, and implementing context-sensitive quality standards that bridge the gap between evidence and routine.

Hip fracture care is becoming increasingly complex

A national cohort study using data from the National Hip Fracture Database examined 735,436 hip fractures in people aged ≥60 years across England, Wales and Northern Ireland.

Key findings:
• Annual hip fracture numbers increased by 13.5%, from 55,832 in 2012 to 63,341 in 2022.
• Overall incidence declined slightly, from 422.8 to 412.8 per 100,000 population.
• Incidence fell in women but increased in men.
• The proportion of patients with ASA grade ≥3 rose substantially in both sexes, reflecting increasing systemic disease burden.
• More patients were living at home before their fracture, increasing demands on discharge planning, rehabilitation and community support.

The authors highlight the need to align prevention strategies and care resources with a growing and increasingly complex hip fracture population.
