Erasmus MC in Rotterdam has a dedicated referral clinic for the assessment and management of aesthetic filler complications with ultrasound.

Patients are referred from across the country and beyond. Every 30 minutes, another complex case: vascular compromise, inflammatory reactions, nodules, misplaced filler, or delayed complications.

One of the biggest lessons is that aesthetic medicine is not only about learning how to inject.

It is about learning how to prevent complications, recognize them early, and know how to manage them successfully when they occur.

One of the most common question I get, does Botox make you age faster ?

And the answer is: no — it actually does the opposite.

Think of it like this:
Every time your muscles contract, they fold the skin — like a piece of paper.
Over time, these folds become lines.

With Botox, we reduce this repetitive contraction for a few months.

As an entrepreneur, the mind is never really off.
There’s always something to improve, plan, or rethink.

Last weekend reminded me of something simple:
it’s not a race — it’s a marathon.

While walking up waterfalls in Austria, we kept telling our son:
Don’t run. Save your energy. One step at a time.
And he made it all the way to the top.

A good reminder for life and work:

This patient had lip filler about 15 years ago and was unhappy with the result.
She came for dissolving and improvement.

Before any treatment, I always check with ultrasound.

What we saw: a typical snowstorm effect — highly suggestive of permanent silicone oil.

Important:
This is not hyaluronic acid filler.
That means hyaluronidase will not work.

Trying to dissolve or even adding new filler can:
• worsen swelling
• trigger inflammation
• create complications
• make the result more unpredictable

In these cases, the options are limited:
• accept the current situation
• or consider surgical removal

This is why ultrasound before correction is essential —
not every “filler” can be dissolved.
