Judgment of advanced stage: refer to the vitiligo disease activity score
(VIDA) [2], clinical features, isomorphic reaction, Wood lamp inspection results. VIDA points: new skin lesions or original skin lesion enlargement "+ 4 points" in the past 6 weeks, new skin lesions or original skin lesion enlargement "+ 3 points" in the past 3 months, new skin lesions or enlargement in the past 6 months Original skin lesion enlargement scale "+ 2 points", new skin lesions or original skin lesion enlargement scale "+1 point" in the past year, "0 points" for at least 1 year stable, at least 1 year stable with spontaneous pigment regeneration meter "- 1 point". The VIDA total score> 1 point is the progression stage, and ≥ 4 is the rapid progression stage. The appearance of blurred edges of skin lesions, inflammatory vitiligo (including pruritus, erythema, etc.), tricolor vitiligo, confetti-like leukoplakia or hypopigmentation spots and other clinical manifestations can be judged as advanced vitiligo [3]. Homomorphic reaction refers to the appearance of leukoplakia within 1 year of skin injury. The injury can be physical (trauma, cut, scratch, mechanical friction, permanent compression, thermal burn, freezing injury), chemical, allergic (allergic contact dermatitis) ) Or other inflammatory skin diseases, irritation (vaccinations, tattoos, etc.), treatment
(Radiation therapy, phototherapy), etc. The Wood light shows that the skin lesions are gray-white in color and the borders are not clear. The area of the skin lesions under the Wood light>the visually measured area indicates that it is an advanced stage. The above VIDA points, clinical characteristics, isomorphic reactions, and Wood lamp inspection results can be considered if any one of them is met
The condition is progressing. Judgment of stable phase: ①VIDA score is 0; ②Clinical features: white spots are porcelain-white, with clear edges or pigmentation;
③No isomorphic reaction (≥ 1 year); ④Wood lamp: The skin lesions are white in color, with clear boundaries. The area of the skin lesions under the Wood lamp is ≤ the visually measured area. If the above conditions are met, it indicates a stable period. Can refer to the image changes of laser confocal scanning microscope (referred to as skin CT) [4] and dermoscopy at the same time, supplemented by diagnosis.
