🔧 Ahmed valve tube erosion repair + Paul Glaucoma Implant placement.

This video shows my surgical approach for managing a tube erosion in a patient whose Ahmed valve was no longer functioning optimally—with an IOP of 24 mmHg despite a previous device revision.

Given the dual challenge of the exposure and the uncontrolled IOP, I decided to explant the exposed Ahmed tube from the anterior chamber and place a Paul Glaucoma Implant in the inferonasal quadrant to achieve better pressure control while completely resolving the erosion area.

🚨 Narrow Angles Alert: Medications That Can Induce Acute Angle-Closure.

Many common medications prescribed daily for migraines, depression, or even a simple cold have the potential to trigger a true ophthalmic emergency: Acute Angle-Closure.

As eye care professionals, keeping the two main pathophysiological mechanisms on our radar is key:
1️⃣ Pupillary Block (Mydriasis): Drugs with anticholinergic or sympathomimetic effects (like nebulized bronchodilators or SSRIs) that dilate the pupil and crowd the angle.
2️⃣ Non-Pupillary Block (Uveal Effusion): Medications like Topiramate or sulfonamides, which induce ciliary body edema, causing a forward shift of the lens-iris diaphragm. Keep in mind: these typically present as acute bilateral closures.

💡 Clinical Takeaway: Performing a timely gonioscopy in suspect patients can prevent irreversible vision loss. Always review systemic medications during your exams.

🇨🇭 Honored to be a finalist at the Swiss International Glaucoma Symposium!

Despite some unexpected challenges that kept me from traveling to Lausanne, I had the incredible opportunity to present my surgical video remotely today.

In this case, I showed the management of a severe hypotony complication after a non-penetrating deep sclerectomy by using an autologous scleral graft when donor tissue wasn’t available.

A huge thank you to the organizing committee for making this remote presentation possible, and to everyone who has supported our clinical and academic journey. The pursuit of surgical excellence never stops! 👁️✨
