Endometriosis can affect fertility through inflammation, adhesions, damage to the fallopian tubes, and sometimes by impacting egg quality or the implantation environment. However, IVF/ICSI is considered one of the most effective treatment options for women with endometriosis, especially in cases of delayed pregnancy, low ovarian reserve, blocked tubes, or previous unsuccessful attempts.
In IVF centers around the world, treatment plans are individualized based on the womanβs age, ovarian reserve, severity of symptoms, presence of endometriomas (ovarian cysts related to endometriosis), male factor infertility, and the number of previous treatment attempts. Surgery for endometriomas is not routinely recommended solely to improve IVF success rates, as surgery may reduce ovarian reserve. However, it may still be considered in cases of severe pain or when access to the follicles during egg retrieval is difficult.
