In 2020, orthopaedic and podiatry organisations (led by a consensus from the American Orthopaedic Foot & Ankle Society) officially replaced the term Tibialis Posterior Tendon Dysfunction (TPTD) with Progressive Collapsing Foot Deformity (PCFD).
The medical community adopted PCFD because the condition involves a complex group of structures. The posterior tibial tendon is only one of many elements (including the spring and deltoid ligaments) that fail as the arch collapses.
Two new papers — a 14-study meta-analysis and a 669-patient cohort — build the strongest orthotics case yet for progressive collapsing foot deformity
A systematic review and meta-analysis (Khaliliyan et al., Foot & Ankle Surgery, 2026) pooled 14 studies on orthotic devices for progressive collapsing foot deformity (PCFD). Orthoses reduced hindfoot eversion by 3–5° and increased forefoot plantarflexion by 6–9°; meta-analysis showed a 56.84% improvement in the Foot Functional Index (95% CI 41.1–72.58) and a 28.53-point increase on the AOFAS Ankle/Hindfoot Scale (95% CI 22.66–34.4). Quality was assessed with ROBINS-I, reflecting that most underlying studies are non-randomised.
Separately, a retrospective cohort just published in the Journal of Foot and Ankle Research (Robb, 2026) examined 669 PCFD encounters across 130,365 appointments at 25 Canadian pedorthic clinics (2016–2025). 80% of patients reported symptom improvement; 56% wore orthoses more than six hours daily, and comfort correlated positively with both wear time and pain reduction. The authors explicitly frame this as groundwork for a future RCT, not proof of efficacy on its own.
