Myofasciotomy explained: a minimally invasive solution for persistent Toe Walking.👣🥼

At our practice, we follow the Pomarino Method, a clear step-by-step approach to retrain walking naturally using pyramid insoles and night splints. In many cases, this allows children to improve gait without surgery.

If the fascia is very tight and orthotics alone cannot achieve proper heel contact, we may consider a high-precision Myofasciotomy. Performed with our specialized scalpel holder through tiny entry points, it minimizes scarring and discomfort while improving foot function.

Our approach:

Milka came all the way from Bolivia with one simple wish: to see her child walk more independently.

Before treatment, every step required a walker. Walking without support wasn’t possible yet, and daily movement was a constant challenge for him and his family.

In our practice, we worked step by step to improve his gait.

The goal was never a “perfect” walk. The goal was progress, more stability, more confidence, and more independence. And then one day, he didn’t need the walker anymore. Is his gait perfect? No. But it is much stronger and much more functional, and most importantly, it allows him to move through the world more independently.

At the end of the video, you see Milka’s reaction. Sometimes emotions say more than any clinical result ever could. Moments like this remind us why foot health matters.

Not All “Flat Feet” Are The Same. 🦶🚫

Most people think a “flat foot” is a structural death sentence. They look for orthotics to passively push the arch up. But in biomechanics, one principle stands above all:
Function > Structure.

A foot can look flat and still function like a high-performance spring. And a foot can appear to have a “perfect” arch but remain unstable under load. What truly matters isn’t the static shape, it’s how well the foot manages kinetic energy dynamically.

Here are four general pillars often used in functional foot training and rehabilitation to improve foot mechanics and restore the natural spring effect:

1️⃣ TENSION (Isolated Inversion)
The foot needs an active anchor. Training the tibialis posterior to stabilize the arch, without gripping with the toes, builds internal support rather than relying on passive structures.

2️⃣ LOAD CONTROL (Forefoot Sensory Input)
The forefoot plays a key sensory role. Improving forefoot awareness and sequential load distribution (sometimes described as the “piano toes” effect) helps transform a rigid forefoot into a responsive structure.

3️⃣ & 4️⃣ RECOIL (The Spring Effect)
A healthy foot works together with the Achilles tendon like a mechanical spring. Through rotation, elastic energy storage, and controlled release, the system becomes efficient and powerful, essential not only for athletes, but for everyday movement.

New Insights into Persistent Toe Walking and SBF1 Genetic Variants 🧬

Is persistent Toe Walking truly „idiopathic,“ or are we looking at a complex genetic landscape?

Our latest publication explores a cohort of 86 children. We investigated the presence of heterozygous SBF1 variants, traditionally associated with autosomal recessive Charcot-Marie-Tooth disease (CMT4B3), and their phenotypic expression.

🔵Key Findings:
Phenotypic Overlap: We observed mild neuromotor and musculoskeletal features (such as pes cavus and lumbar hyperlordosis) that partially overlap with reported CMT4B3 phenotypes.
Clinical Presentation: Muscle weakness and reflex abnormalities were less frequent than in recessive cases.

The „VUS“ Challenge: Most identified variants remain classified as Variants of Uncertain Significance (VUS), highlighting the need for longitudinal follow-up and segregation studies to clarify their clinical relevance.
This research moves us closer to understanding whether these variants represent a gene-dosage effect or a specific modifier in neuromuscular development.
