Diagnosis of hallux valgus
Hallux valgus means ball of the big toe and is one of the most common foot deformities. Women are more affected than men. The clearly visible outer bulge on the toe set of the big toe is caused by the shift in the direction of the middle toe
What is hallux valgus?
All toe bones on a healthy foot are facing straight ahead. Hallux valgus, on the other hand, manifests itself as an internal rotation of the big toe. The base joint is shifted outward at the attachment, which can be seen as a more or less large bulge on the outside.
Hallux valgus is usually inherited over the generations. It's a genetic predisposition. The manifestation of the malalignment is favored by external circumstances. Wearing high-heeled shoes as well as shoes that are generally too tight and weak connective tissue, but also rheumatism, obesity and a professional activity that is mainly carried out while standing can influence the progression.
Hallux valgus doesn't just change the look of the foot. The misalignment leads to damage to the metatarsophalangeal joint of the big toe. Cartilage wear and tear, including osteoarthritis, is associated with severe pain. A specialist should therefore be consulted at the early stage of the toe shift in order to keep the consequential damage to the joint as low as possible.
What are the symptoms of hallux valgus?
The onset of hallux valgus is heralded by pain in the middle toes, widening of the forefoot and excessive callus formation on the underside of the foot. Calluses and corns are not uncommon. The change can be clearly seen in the outer bulge at the base of the metatarsophalangeal joint of the big toe. Unfortunately, it is not a purely cosmetic matter; in the long term, the natural function of the foot is impaired. There is swelling and redness on the ball of the foot, which can lead to inflammation. A specialist should therefore be consulted as soon as the first pain in the toes appears.
How do you diagnose hallux valgus?
Hallux valgus is usually easy to diagnose visually; the suspicion is confirmed by palpating the joint. An X-ray taken while standing under load reveals the extent of the deformation.
How do you treat hallux valgus conservatively?
The malalignment in hallux valgus can be flexible or fixed. The flexible shape allows the big toe to be moved to its actual position. Conservative treatments can promise success here. If the misalignment has fixed itself or if there is severe pain and pressure points, only surgical intervention can help.
How do you treat hallux valgus surgically?
Over 100 surgical techniques are known for the surgical treatment of the ball of the big toe.
Reverdin rearrangement operation
The Reverdin conversion operation is used for mild to moderate malpositions. It is a joint-preserving procedure in which the bone is fixed with a bioresorbable implant that does not require any additional surgery to remove metal. After the wound has healed, a therapeutic shoe is used; the additional use of forearm crutches is not required.
Basic conversion operation
Severe misalignments of the metatarsophalangeal joint of the big toe are corrected in an operation close to the base, which is carried out in combination with a soft tissue intervention. An angle-stable plate made of titanium is used, which makes it possible to load the foot in the therapeutic shoe immediately.
Lapidus arthrodesis
In its extreme form, hallux valgus is accompanied by instability in the passage between the metatarsus and the tarsus. Therefore, a stabilization of the first joint of the tarsus according to Lapidus in conjunction with a soft tissue intervention is necessary.
Akin osteotomy
The classic hallux valgus is often accompanied by another deformity: the hallux interphalangeus. This is an inward curvature of the big toe at the level of the base phalanx. The Akin osteotomy, which corrects the curvature by removing a wedge from the base phalanx, is an ideal therapy.
How long does the rehabilitation take for hallux valgus?
Your stay usually lasts 3-5 days and depends on the surgical procedure. Although you can put weight on your foot immediately after the procedure, you should avoid driving for 6 weeks. A therapeutic shoe helps you walk. No crutches are required. After the therapeutic shoe phase, you can wear normal shoes again, but they shouldn't be uncomfortable and shouldn't be too small. You can also wear open-toed shoes because the surgical scars heal almost completely. In principle, hallux valgus cannot recur after a successful operation.
Reverdin
