Tests you need to pass to work as a licensed doctor in Germany

Only qualifying exams, and no entrance exam.

One of the biggest cultural shifts doctors experience when moving to Germany isn’t just the language.

It’s the work culture.

In India, many doctors grow up in systems where:
• Extra hours are expected
• Weekend work is normal
• Night duties are exhausting
• Overtime is rarely compensated properly

And questioning it? Not encouraged.

In Germany, it works differently.

As a doctor in Germany, every extra hour:
✔️ Is documented
✔️ Is compensated
✔️ Or converted into time off
✔️ Night shifts & weekends are paid extra
✔️ There are strict limits on maximum working hours

That doesn’t mean the job is easy.
Residency is still intense.
Responsibility is still huge.

But the structure protects you.

And for many foreign doctors, that financial and professional respect is one of the most surprising (and relieving) parts of the transition.

As I return from India and reflect on this recent trip, I feel deeply grateful for the opportunity to engage, share, and listen. What began as a journey back home became a moment of pause—one that allowed me to reconnect with purpose and perspective.

Returning to where it all began, speaking at my alma mater, UCMS Delhi, wasn’t just a talk, it was a reminder of why RBT exists in the first place. Meeting students who are standing exactly where I once stood brought fresh perspective, honest questions, and a renewed sense of responsibility. Conversations like these ground our work and remind us who we’re really building for.

Equally meaningful was speaking at ESIC medical college in Hyderabad. Engaging with aspirants from different backgrounds; each carrying their own doubts, ambitions, and expectations—was both insightful and energising. Their questions went beyond processes and pathways; they reflected deeper concerns about direction, clarity, and making informed decisions early on. Listening to them helps us see where guidance is still missing and where we can do better.

As we step into the new year, the intention is simple: to keep listening, keep improving, and keep refining how we support aspiring doctors. Grateful for the conversations, the curiosity, and the chance to do this work with greater clarity, care, and responsibility.

Short answer: apply everywhere.
Long answer: here’s why

When it comes to applying for residency in Germany, there isn’t one perfect hospital, city, or strategy that works for everyone. What does work—consistently—is casting a wide net.

Most aspirants start by targeting big cities like Berlin, Munich, or Hamburg. And yes, they’re attractive: international vibe, good infrastructure, and familiarity. But they’re also highly competitive. Hospitals there receive hundreds of applications for the same positions, which means longer waiting times and more rejections—often despite strong profiles.
Smaller cities and peripheral regions, on the other hand, are actively looking for doctors. The demand is real. Many hospitals there are more open to international graduates, quicker with responses, and more willing to offer Hospitation → contract pathways. What you might “lose” in city buzz, you often gain in hands-on learning, supportive departments, and faster career progression.

That’s why the smartest approach is this:

Apply to big cities

Apply to small cities

Apply to academic hospitals

Apply to regional hospitals

Apply even if you think, “This might be a long shot”

Your first residency job in Germany doesn’t have to be perfect. It has to be strategic. Once you’re in the system, moving cities or hospitals becomes far easier than trying to enter it from the outside.

Maybe the next plot twist really is that everything works out — not loudly, not overnight, but steadily, in ways you couldn’t have planned for.

A new year has a quiet way of doing that.

It doesn’t arrive with guarantees, only with possibilities.

New beginnings rarely look dramatic at first — sometimes they look like doubt, waiting, starting again, trusting yourself one more time.

For many doctors, this chapter begins far from familiar systems, languages, and comfort zones. It begins with questions, paperwork, long timelines, and the courage to believe that the effort will eventually make sense.

At RBT, we see this moment up close — the in-between phase before things fall into place. The uncertainty before the yes. The patience it takes to build a medical career in Germany step by step, even when the outcome still feels distant.
