Can an injection meaningfully support cartilage-preserving surgery?

This 2026 systematic review included 5 randomized controlled trials with 217 patients undergoing subchondral drilling or microfracture for focal knee chondral defects. The adjuncts studied were PRP, peripheral blood stem cells and adipose-derived cell products.

In the PRP subgroup, pain improved by 0.93 points on the VAS and IKDC by 13.4 points. The pain effect, however, remained below the reported 1.5-point threshold for a clinically important change. Cell-based protocols showed more consistent structural signals on MRI, histology and arthroscopy, but the products, dosing schedules, surgical procedures and rehabilitation protocols differed substantially.

The key message is measured: orthobiologics may become useful adjuncts in selected protocols, but they do not replace a well-selected cartilage-preserving procedure. With only 5 heterogeneous trials - including 3 with bias concerns and 1 at high risk of bias - no specific clinical recommendation can yet be made.

A highly productive and inspiring exchange at the Strategy Meeting of the DGOU, held on March 27–28 in Seeheim-Jugenheim. Led by the presidents and secretaries general of the DGU and DGOOC, the meeting focused on key future topics in orthopaedics and trauma surgery. In an open and collaborative atmosphere, we engaged in in-depth discussions on major challenges and strategic developments—from ambulatory care transformation to training and future healthcare structures. A valuable platform shaping the future of our field. .ev

The Orthopaedic Film Festival on Winglet has once again demonstrated its unique value—now in its 5th edition. What started as an innovative concept has evolved into a well-established format in global orthopaedic education. Particularly exciting is the increasing international reach, with outstanding contributions from across different continents, enabling a truly global exchange of knowledge. A strong example of how digital education continues to transform our field.

The Sports Orthopaedics Course in Munich has been a key highlight in my calendar for several years. Outstanding scientific presentations, high-quality content, and—most importantly—engaging discussions with participants and colleagues make this course truly special. It is the combination of evidence-based education and dynamic exchange that creates a learning experience well beyond traditional formats. Grateful to be part of this inspiring course once again. Danke für die Einldung .mehl.ortho
