What does the SERENA-6 trial mean for everyday clinical practice? At the GAWC Webinar 2026, Prof. Dr. Nadia Harbeck discussed this pivotal study presented at ASCO, highlighting a crucial topic: the gap between a strong clinical trial signal and its broad implementation in routine patient care.

🧬 The SERENA-6 Strategy
The trial evaluated whether liquid biopsy can detect emergent ESR1 mutations during first-line AI + CDK4/6 inhibitor therapy before radiologic progression, allowing for an early switch to camizestrant.

📊 How the Audience Voted
We asked our international audience if they would screen all patients for molecular progression via liquid biopsy if routine care reimbursement were available. With nearly 300 participants voting, the responses were highly nuanced:

60% would use liquid biopsy only in selected patients.
25% would use it for all patients.
10% were not convinced by the current data.
5% wanted to reflect further after the webinar.

The results clearly underline how much discussion is still needed regarding clinical relevance versus real-world implementation.

The discussion centered on the rapidly advancing therapeutic landscape and the clinical necessity of personalized treatment strategies:

• Prof. Harbeck underscores the importance of testing for ESR1 and PIK3CA mutations, noting that resistance can emerge dynamically during treatment.

• The Role of Liquid Biopsies: Utilizing liquid biopsies after each line of therapy is vital to identifying new mutations and adapting patient care in real-time.

• The Shift in Standard of Care: A look at how Antibody-Drug Conjugates (ADCs), specifically T-DXd, are increasingly replacing traditional chemotherapy in HER2-low and ultra-low disease settings.
