🫁 Chronic Obstructive Pulmonary Disease (COPD)
Chronic Obstructive Pulmonary Disease (COPD) is a chronic respiratory disease characterized by progressive airflow limitation that is not fully reversible. It causes breathing difficulties and significantly affects quality of life. COPD mainly includes chronic bronchitis and emphysema.
🔹 Symptoms
Shortness of breath, especially during physical activity.
Chronic cough, with or without sputum production.
Wheezing.
Chest tightness or discomfort.
Recurrent respiratory infections.
Fatigue and reduced physical activity.
Weight loss in advanced stages.
Swelling of the feet or ankles in severe cases.
Acute exacerbations, during which symptoms suddenly worsen and may require hospitalization.
🔹 Causes and Risk Factors
🚬 Smoking is the leading and most important cause of COPD, including exposure to secondhand smoke.
Other risk factors include:
Occupational exposure to dust, fumes, and chemicals.
Air pollution.
Alpha-1 antitrypsin deficiency (a rare inherited condition).
Increasing age.
Asthma, particularly when poorly controlled.
🔹 Complications
Recurrent lung infections.
Respiratory failure.
Pulmonary hypertension.
Heart disease.
Increased risk of lung cancer.
Depression and reduced quality of life.
🔹 Diagnosis
Diagnosis is based on:
✅ Medical history and physical examination.
✅ Spirometry (pulmonary function testing), the key test for confirming COPD.
✅ Chest X-ray or chest CT scan when indicated.
✅ Measurement of blood oxygen levels.
✅ Blood tests, including screening for alpha-1 antitrypsin deficiency in selected patients.
🔹 Treatment
Although COPD has no cure, appropriate treatment can slow disease progression, relieve symptoms, and improve quality of life. Treatment may include:
Smoking cessation.
Bronchodilator inhalers.
Inhaled corticosteroids in selected patients.
Pulmonary rehabilitation and regular exercise.
Long-term oxygen therapy for patients with chronic hypoxemia.
Antibiotics when bacterial respiratory infections are present.
Lung volume reduction surgery or lung transplantation in carefully selected advanced cases.
🔹 Prevention
✔️ Avoid smoking and exposure to secondhand smoke.
✔️ Use appropriate protective equipment in occupational settings.
✔️ Receive an annual influenza vaccine and pneumococcal vaccination as recommended.
✔️ Engage in regular physical activity.
✔️ Seek medical evaluation for persistent cough or shortness of breath, especially in smokers.
📌 Key Message: Every smoker over the age of 40 who experiences chronic cough or shortness of breath should undergo spirometry.
Early diagnosis and smoking cessation are the most effective ways to preserve lung function, reduce disease progression, and improve quality of life.
