مريض بيجيله palpitations على نوبات. وهو بين النوبات، الـECG فيه short PR interval وبداية الـQRS فيها delta wave.

السؤال: الـimpulse وصلت للـventricle بدري منين؟
A) من الـAV node نفسها
B) من accessory pathway بيتجاوز الـAV node

اختار قبل ما تكمل 👀

الإجابة: B) Accessory pathway.

في Wolff-Parkinson-White syndrome فيه accessory pathway، اسمه bundle of Kent، يوصل الـimpulse من الـatrium للـventricle بعيدًا عن الـnormal AV nodal delay.

جزء من الـventricle يتنشّط مبكرًا، فـPR interval يقصر، وأول الـQRS يظهر فيه slurred upstroke اسمه delta wave. ومعاه الـQRS يبقى أعرض في الـbaseline ECG.

ليه اختيار الـAV node مغري؟ لأنها الـnormal pathway للـimpulse. لكن دورها الطبيعي إنها تعمل conduction delay، مش pre-excitation.

القاعدة الذهنية: short PR + delta wave، دور على accessory pathway سبق الـAV node.

في suspected ectopic pregnancy، الـbeta-hCG لوحده مش كفاية، والـultrasound لوحده مش كفاية.

في EL Husseiny's Essentials for USMLE step 2 CK Obstetrics، جزء Ectopic Pregnancy، التشخيص بيعتمد على ربط quantitative serum beta-hCG بالـtransvaginal ultrasound.

الفكرة:
• لو الـbeta-hCG وصل للمستوى اللي المفروض يظهر عنده intrauterine pregnancy على TVUS، ومفيش gestational sac داخل الرحم، يبقى ectopic pregnancy احتمال قوي.
• لو الـbeta-hCG لسه أقل من المستوى ده ومفيش IUP ظاهر، متستعجلش التشخيص.
• كرر quantitative beta-hCG والـTVUS بعد 2 إلى 3 أيام لحد ما الصورة تبقى أوضح.

نقطة Step 2 CK:
متفسرش رقم beta-hCG في فراغ.
اقرأه دايمًا مع نتيجة الـtransvaginal ultrasound.

الخلل الأقرب فين؟
A) Lysosomal trafficking and fusion
B) Oxidative burst

اختار قبل ما تكمل 👀

الإجابة: A) Lysosomal trafficking and fusion.

الصورة دي ترجّح Chédiak-Higashi syndrome. خلل جين LYST يربك lysosomal trafficking والـphagosome-lysosome fusion، وده يفسّر ظهور giant granules في الخلايا وتكرار العدوى. الـpartial albinism تفصيلة بتقوّي الترجيح.

طب ليه oxidative burst اختيار مغري؟ لأن خلله كمان يضعف microbial killing، زي ما يحصل في chronic granulomatous disease. لكن giant granules مع partial albinism هنا بتوجّهك لمسار lysosomal trafficking.

القاعدة الذهنية: giant granules + partial albinism، فكّر في Chédiak-Higashi.

في suspected aortic dissection، السؤال مش بس: أنهي test دقيق؟
السؤال الأهم: المريض stable ولا لأ؟

في EL Husseiny's Essentials for USMLE step 2 CK Internal Medicine، جزء Aortic Dissection، الـCTA والـMRA والـTEE كلهم اختبارات دقيقة جدًا لتأكيد التشخيص.

لكن الاختيار بيتغير حسب حالة المريض.

لو المريض hemodynamically unstable، أو عنده renal insufficiency أو contrast allergy، فالـtransesophageal echocardiography (TEE) يبقى الاختيار المفضل المذكور في الكتاب.

ليه؟
لأنه يقدر يوضح الـintimal flap من غير ما تعتمد على نقل مريض غير مستقر للـCT scanner أو contrast iodinated.

نقطة Step 2 CK:
متختارش الاختبار من اسمه بس.
اختار الاختبار المناسب لحالة المريض، خصوصًا لو unstable.

مريض عنده hypokalemia وmetabolic alkalosis، وضغطه مش عالي. في التحاليل لقيت low urinary calcium.قبل ما تكمل: تختار Bartt...

قبل ما تكمل: تختار Bartter ولا Gitelman؟ 👀

A) Bartter syndrome
B) Gitelman syndrome

الإجابة: B) Gitelman.

الـhypokalemia والـmetabolic alkalosis موجودين في الاتنين، فلو اعتمدت عليهم بس مش هتقدر تفرق. الـfinding اللي غيّر الإجابة هنا هو low urinary calcium.

في Gitelman، الخلل في NaCl reabsorption في الـdistal convoluted tubule. الصورة بتشبه تأثير الـthiazide: urinary calcium يقل، وغالبًا يحصل hypomagnesemia كمان.

أما Bartter فالخلل في الـthick ascending limb of the loop of Henle عند الـNa-K-2Cl cotransporter. ده يشبه تأثير loop diuretics، والـurinary calcium فيه يزيد.

ليه Bartter اختيار مغري؟ لأنه برضه يعمل hypokalemia وmetabolic alkalosis. لكن low urinary calcium هو اللي يرجّح Gitelman في السؤال ده.

افتكرها كقرار واحد:
لو urinary calcium قليل مع الصورة دي، فكّر في Gitelman.
لو urinary calcium عالي، فكّر في Bartter.

📝 Before Starting Your NBME

Your NBME is not just about testing your knowledge; it’s also about training your focus, stamina, and test-taking strategy.

1. Before You start

• Take the assessment when you are well-rested and mentally alert.
• Sleep well the night before your NBME.
• Start your NBME at around 9 AM, just like your actual exam day.
• Simulate real exam conditions as closely as possible:
• DO NOT CHECK YOUR ANSWERS OR SCORES BETWEEN BLOCKS.
• Do not review or discuss previous questions during breaks.
• Do not let uncertainty about a previous block affect your performance on the next one.
• Stay focused on the current block and treat every new question as a fresh start.
• Choose a quiet environment, silence your phone, and eliminate distractions.
• Do not use notes, Google, Anki, AI, or any other resources during the assessment.

2. Time Management

• 10 blocks × 20 questions = 200 questions
• 30 minutes per block = 5 hours of testing
• 1 additional hour for breaks divide it as you prefer, but do not exceed the allocated time.
• Total duration: 6 hours

3. During Breaks

• Step away from the screen.
• Drink water and eat a light snack if needed.
• Avoid reviewing previous questions or calculating your expected score.
• Do not let a difficult block affect your concentration on the next one.
• Return to the next block with a fresh mindset.

Final Reminder

Your job is not to change the answers you already submitted in your mind. Your job is to maximize your performance on the questions in front of you.

One block at a time. One question at a time. Stay focused and trust your preparation.

في suspected osteomyelitis، الـX-ray مهم كبداية، لكنه مش نهاية الـworkup لو الشك لسه عالي.

في EL Husseiny's Essentials for USMLE step 2 CK Internal Medicine، جزء Osteomyelitis، الـinitial workup يشمل:
• complete blood count
• blood cultures
• ESR وCRP
• plain X-ray

لكن في suspected vertebral osteomyelitis، الـMRI هو imaging modality of choice، وكمان يقدر يوضح epidural abscess أو cord compression لو موجودين.

ولو محتاج أدق microbiologic diagnosis، فالـCT-guided aspiration and culture من الـdisc space أو bone هو الأكثر diagnostic، لكنه أكثر invasive.

نقطة Step 2 CK:
ابدأ بالـbasic workup، لكن متقفش عند X-ray لو الصورة السريرية لسه مقلقة.
الـMRI هو الاختبار الأهم لتقييم vertebral osteomyelitis.

ليه مريض عنده recurrent cough وpurulent sputum، والـbronchi عنده متوسعة بشكل دائم؟

البداية recurrent infection مع impaired airway clearance. الـinflammation يستدعي neutrophils، والـneutrophils تطلق elastase بكثرة؛ فيساهم في bronchial wall destruction. عشان كده الـbronchial dilation هنا anatomical ودائم، مش مجرد narrowing بيروح لما الـinflammation يهدأ.

كتاب First Aid for the USMLE Step 1 2026 بيرتب الطريق من infection وضعف الـclearance للـinflammation والـpermanent bronchial dilation. وكتاب EL Husseiny's Essentials for USMLE step 1 Respiratory System بيضيف الحلقة اللي تشرح تلف الـbronchial wall: neutrophil recruitment وإفراز الـelastase.

لو السؤال جمع recurrent infections، daily purulent sputum، وdilated bronchi في الـimaging، اربط الأعراض بالـongoing bronchial wall destruction.
