One of the most challenging hernia cases I have ever encountered.
A 45-year-old female presented with a giant lumbar incisional hernia that had been progressively enlarging over 10 years following left kidney donation surgery. The hernia sac contained the entire stomach, spleen, portions of the left colon and small bowel, as well as the tail of the pancreas.
Our surgical strategy was to place the mesh in the preperitoneal space to minimize morbidity and reduce the risk of adhesions in the event of future recurrence.
After meticulous and extensive dissection, a double-layer preperitoneal mesh was inserted and securely fixed to the ribs superiorly, the iliac crest inferiorly, the linea alba anteriorly, and the psoas muscle posteriorly. An additional onlay mesh was then placed and fixed over the anterior abdominal wall musculature to reinforce the repair.
The postoperative course was uneventful. Follow-up CT imaging at one month demonstrated an intact repair with no evidence of recurrence, and the patient remains recurrence-free clinically at 3 months.
